Established in 1868, St John's Residence and School for Boys, a dedicated social service institution with a caring work culture is seeking a well-meaning and engaging Aftercare Permanency Specialist to work with our residents, boys ages 11 - 21 .
WORK-LIFE BALANCE: This is a 35-hour week position, with on-call responsibilities. St. John's offers a stress-free work environment and competitive benefits to all full-time staff after 60 days of employment. We offer 3 weeks' vacation with this position.
QUALIFICATIONS: . HS Diploma required; College degree strongly preferred. Direct experience working with troubled/at-risk teens and their families in a foster care system is critical. Experience accessing governmental and community-based supports/services also essential.
JOB DESCRIPTION: . The Aftercare Permanency Specialist is the Residential Treatment Center's primary Aftercare Worker. Establishes healthy relationships with youth while in care, and with families prior to discharge date. Assumes a caseload of youth and their families at approximately 4 months prior to discharge. Maintains associated responsibilities through to Final Discharge. Be a master youth group worker, grounded in insight.
CORE RESPONSIBILITIES:
Shares a caseload of youth scheduled to "return to Parent/Family" with the assigned Permanency Specialist from (approximately 4 months prior to discharge) through to final Discharge
Works with the assigned Permanency Planners to assess youth and parental circumstances and capabilities to determine readiness for reunification.
Provides regularly scheduled (weekly) group meetings (a version of "Becoming A Man") with assigned youth.
Provides one-on-one counseling to assigned youth (in person/on phone/by email) as needed.
Provides intensive supports to families to ensure access to material supports (housing, jobs, childcare, transportation, homemaking services), as appropriate.
Provides intensive support to assist youth and families to more effectively engage and navigate public services and resources (housing, health/behavioral healthcare and public assistance), as needed.
Works intensively with youth and families to match youth with best possible community-based options.
Monitors and support those youth as they pursue their employment plans/goals.
Works with youth and families to develop/nurture their own social support so as to enhance
Works with parents(s) to enhance their "executive functions" (healthy family routines and activities, positive parent-youth interactions, enlightened behavior management, etc.)
Provides real time in-person and/or on-phone emergency crises intervention support for assigned (discharged) youth - 24/7.
Special duties as assigned.
