CP Nassau is an independent, not-for-profit health agency serving over 1800 children and adults with cerebral palsy, developmental and other disabilities. The Association, founded in 1948, operates a comprehensive Treatment and Rehabilitation Center in Roosevelt, New York, which has earned international recognition for innovative and high-quality programs.
We seek a caring professional to join our team with following qualifications:
Bachelor?s Degree experience with DD population
Knowledge of ADA regulations and accessibility requirements
Valid NYS Driver?s License
AMAP Certification
Prior community experience preferred
The qualified applicants will be responsible for the following:
Identify, develop and coordinate sites within community for consumers consistent with consumer?s needs, desires and valued outcomes.
Coordinate planning of activities at sites with community coach as per consumer?s needs, desires and valued outcomes.
Assess sites for wheelchair accessibility.
Develop community contacts for recruitment for in-house services, including, but not limited to, per diem and specialty staff.
Ensure needs of consumers are met including but not limited to their safety and well-being.
Ensure compliance with OPWDD/OPTS regulations and Day Habilitation Program Policy & Procedure, to include but not limited to training on behavioral plans/strategies & protocols.
Maintain daily log of services provided in the community.
Drive consumers to community integration experiences.
Incorporated Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.
