Client Navigator Job Opening in New York, New york - Dennelisse Corporation is currently seeking full-time Client Navigators. Client Navigators assist the Case Managers with coordinating services for the client. The Client Navigator will also have the responsibility to provide health information to the client and ensure that the client is compliant with the Health Home include:
Assist the Care Coordinator by providing case management for clients and their families.
Conduct field work 60% of the time (which includes home, hospital, governmental offices and other provider visits.
Conduct client screenings and intakes (as needed).
Ensure Care plan implementation and client goal attainment.
Promote empowerment, self sufficiency and independence in clients, through advocacy, teaching, modeling, and education.
Assist in training and educating clients and their collaterals on health management, appropriate medical and social behavior changes and healthy lifestyle choices.
Attend weekly team meetings.
Follow-up with Care Coordinators
Facilitate client needs in the absence of a Care Coordinator or other team member.
Accompany clients to various healthcare provider, social services or housing appointments.
Including but not limited to any other duties assigned by the Care Coordinator, immediate Lead Care Coordinator or Director of Care Coordination.
:
At minimum a High School Degree with minimum 2 years of experience in the healthcare field OR 3 years experience in customer service.
Associates Degree in health, education or human services and one year of qualifying experience working with HIV/AIDS or any other chronic illness, disease management, substance abuse, homelessness or needy families (preferred);
Excellent written and oral communications skills
Computer literate
Bi-lingual a plus.
