Medical Center Liason Job Opening in Flint, Michigan - Job Title: Medical Center Liaison
Accountable: CEO/Director of Programs
Essential Duties and Responsibilities
Works as part of interdisciplinary team to achieve optimal health outcomes for patients, such as viral load suppression.
Recruit additional medical providers, specialists, and medical facilities as well as community providers to join the network to address needed or missing services from the continuum of care.
Provide medical oversight to interdisciplinary team through assisting in interpreting lab results, medical records, and sharing information from medical providers.
Facilitate communication about patient management between primary care providers and Care Coordinators (Case Managers) during intake/enrollment activities as well as on an ongoing basis.
Forwards informational reports (e.g., appointment dispositions, laboratory results, etc.) produced by clinical information systems to Care Coordinators.
Participates in the generation of and updates the medical/clinical sections of the Comprehensive Care Plan and forwards to Care Coordinators.
Collaborates with Care Coordinator on medical details of the Comprehensive Care Plan.
May conduct selected center-based Care Coordination activities such as health promotion for low-intensity PLWH, as needed.
Collaborates with the clinical team to manage care coordination resources and target the neediest PLWH for service.
Assessment of health and psychosocial needs, including health literacy, and development of a case management plan collaboratively with all stakeholders.
Planning with all stakeholders to maximize healthcare responses, quality, and cost-effective outcomes.
Facilitating communication and coordination among stakeholders, involving the patient in the decision-making process in order to minimize service fragmentation.
Educating the patient and all stakeholders on treatment options, community resources, insurance benefits, and psychosocial concerns so that timely and informed decisions can be made.
Empowering the patient to problem solve by exploring care options and alternative plans, when necessary, to achieve desired outcome.
Encouraging the appropriate use of healthcare services and striving to improve the quality of care and maintain cost-effectiveness on a case-by-case basis.
Assisting the client in safe transitions of care to the next most appropriate level.
Striving to promote patient self-advocacy and self-determination.
Advocating for both the patient and stakeholders, to facilitate positive outcomes. (However, if a conflict arises, the patient must be the priority.)
Maximize the health of patients by increasing healthcare education that promotes wellness.
Proactively identifying problems and needs, and implementing services that provide appropriate, high-quality care to meet the individualized needs of the patient and his or her family.
A combination of hours, which will vary based on what is needed for the patients/outreach events.
Requires valid Driver?s license, proof of auto insurance, and reliable transportation, where needed to perform duties of the position, as in driving to healthcare locations/meeting across the Wellness Care Coordination Network.
