RN Clinical Coordinator (Home Health) Job Opening in Boulder City, Nevada - GENERAL PURPOSE :
The RN Clinical Coordinator is responsible for coordinating all treatments into the total nursing care plan for patients, participates in patient and family teaching and provides leadership by working cooperatively with ancillary departments and other patient team personnel in maintaining standards for professional nursing practice in the home health setting.
ESSENTIAL FUNCTIONS:
Ability to effectively care for patients. Must possess a current Nevada Driver License along with a vehicle to provide transportation. Ability to lift, walk, carry and stand.
DUTIES AND RESPONSIBILITIES :
Provides care and case management of the acute and sub-acute ill patient, chronically ill and aged convalescent patients in the home setting.
Able to perform and supervise all aspects of case management.
Works under the direction of the Home Health Director to oversee, manage, training and assist nurses and other staff within the department.
Works with the Home Health Director to facilitate and maintain lines of communication with the various ancillary departments utilized by the department.
Maintains the standards of nursing care and implements the policies and procedures of the agency and hospital.
Assesses the patient?s condition and nursing needs; sets goals; prescribes appropriate nursing actions to meet those goals and the physical, psychological, social and rehabilitative needs of the patient, including discharge planning.
Participates in team conferences in development of individualized nursing care plans as needed.
Assists Home Health Director with departmental in-services for the various staff of the department.
Reports any significant change inpatient or home situation to the patient?s primary nurse and /or Home Health Director.
Collaborates with MSW and team when problems arise.
Responsible for maintaining current knowledge of OASIS and Medicare guidelines, and is able to complete paperwork accurately and promptly per home health policy/procedure.
Leads the care team in the care planning process.
Conducts and directs case conferencing with staff on a minimum of weekly basis.
Assists with taking referrals, insurance authorizations, data entry, paperwork processing and chart auditing process, i.e., assists to audit records for completeness weekly.
Identifies and works with patient?s physician and interdisciplinary team in solving medical problems.
Develops measurable realistic care plan from physician orders and patient observation.
Participates in the on call rotation status, which may include weekends and holidays and alteration in the weekday schedule to accommodate on call coverage as needed.
Assists Home Health Director with community relations and community education activities relating to both hospital and home health activities.
Completes annual/biannual hospital and licensure requirements.
Serves as RN in charge of department in the absence of the Home Health Director.
Assumes responsibility to secure office at end of day in the absence of office assistant: duties to include but not limited to knowledge of securing computers, forwarding to answering service , turning off equipment, securing medical records, locked cabinets and agency doors.
Performs other reasonably related duties as assigned by immediate supervisor and other management as required.
SKILLS AND ABILITIES REQUIRED :
Demonstrates the ability to form a good working relationship with other people.
Extensive walking, standing and lifting.
Must have a current Nevada Drivers License.
Must be available for assigned duties, a dependable vehicle with current Nevada registration, which is insured in compliance with State Statutes and has a medical clause.
Will have the ability to follow and carry out instructions and procedures assigned by the Home Health Director.
Will attend inservice classes and continuing education seminars to keep current with the latest nursing techniques, and maintains CEU requirements.
EDUCATION AND EXPERIENCE REQUIRED :
Current licensure by Nevada State Board of Nursing.
Two (2) years of hospital nursing experience preferred.
