RN Case Manager Job Opening in Chesapeake, Virginia - We are currently seeking a full time Registered Nurse (RN) Case Manager for Chesapeake, VA program. The RN Case Manager works as an intricate part of the clinical team where the primary responsibility is to coordinate the overall plan of care for each patient in conjunction with the attending physician.
Qualifications:
The RN Case Manager will possess excellent communication skills (written and verbal) as well as strong interpersonal and organizational skills. Hospice experience is preferred . He/she will be a self-motivated individual who works well in a team environment and is able to work flexible hours (and ?off" hours) as may be needed.
Requirements:
Must be a Registered Nurse with current license in VA
Graduate of NLN accredited school of nursing
Must have at least 2 years of Hospice experience
