Roane General Hospital in Spencer, WV a Critical Access Hospital with 35 SNF/ICF beds, 25 acute care beds, Level IV Trauma Emergency Department, three Rural Health Clinics and a walk-in clinic is currently seeking applicants for a Full-Time Patient Care Navigator For Roane General Medical Clinic (RGMC).
Qualifications
Current clinical license (CMA/LPN) or a minimum of three years clinical experience required,
Previous experience in caring for chronic disease patients required.
Experience in clinical or community health settings preferred.
Previous Care Coordination or Case Management preferred.
Demonstrates evidence of essential leadership, communication, education, collaboration, and counseling skills.
Proficient in various communication technologies.
Effective organizational skills and ability to maintain accurate notes and records.
Previous experience with health IT systems and data reports preferred.
Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with a variety of populations preferred.
Ability to identify and implement appropriate patient communication strategies and overcome accessibility barriers, as required.
Orchestrate the effective delivery of care in accordance with third-party payer program requirements and resources.

Position Summary
Coordinates multidisciplinary care to provide health services to individuals, through effective partnerships with patients, caregivers, families, community resources, and their physician. Facilitates navigation within a patient centered model and across settings to achieve coordinated care that is efficiently provided in an optimal manner. The ability to multitask and work in a fast paced clinic setting is essential.
