Under the supervision of the Associate Director of Linkage, the Linkage Specialist delivers Linkage services to newly diagnosed HIV positive individuals and to positive individuals who have fallen out of care, and performs outreach to build community partnerships facilitating optimal transitions from HIV testing to HIV medical care. The Linkage Specialist will exhibit a strong understanding and experience in the following: * Delivery of outpatient medical care services to persons living with HIV including, opportunities for client education and clinic and patient flow * Factors that influence HIV status disclosure * Delivery of partner counseling and referral services particularly the dual referral and provider referral modalities * Factors that influence successful partner elicitation * The delivery of HIV counseling and testing services particularly rapid testing services * Barriers to engagement in HIV outpatient medical care * The Stages of Change Theory of Health Behavior and factors that influence an individual?s motivation to make a health behavior change in his/her life * Motivational Interviewing and Strengths-based counseling techniques * The Ryan White CARE Act and other payer sources for persons living with HIV/AIDSThe Linkage Specialist will: * Assess clients? understanding and acceptance of their HIV status, applying principles of the Stages of Change Theory of Health Behavior, Motivational Interviewing and strengths-based counseling techniques * Be available to meet and/or speak with clients at their convenience to discuss their HIV status, barriers to entering into HIV outpatient medical care and other services * Deliver all aspects of Linkage services including referral, provisional eligibility determination, assessment, and evaluation of consumer needs/services * Work with AHF Client Retention Coordinators, Benefits Counselors, Office Administrators and Nurse Case Managers to ensure clients are directly linked to care and remain in care * Be available to staff the Linkage On-call Hotline, which is available to clients and HIV testing organizations to call during regular and non-traditional business hours * Represent AHF mission and core values to staff, stakeholders, collaborative associates and community groups * Create an outreach plan targeting specific areas or populations within the region to increase the number of clients receiving AHF linkage services * Prepare presentations on the Linkage program internally and to community organizations and planning bodies to facilitate strategic partnerships for client referrals * Establish and maintain relationships with case managers, HIV testing agencies, community based organizations, etc? * Make contacts/calls on a routine basis to build relationships with potential referral sources * Independently manage a complex schedule of client meetings and clinic appointments, administrative duties such as client case notes, responding to calls from HIV testing staff with newly diagnosed individuals, required staff meetings and trainings * Ensure that 100% of newly enrolled HIV-positive clients are offered PCRS through medical outpatient services * Serve as a liaison between the agency and STD Program-based PHI/DIS for clients choosing the "provider referral" PCRS option * Give input in the development of national Linkage policies and procedures * Participate in the orientation and training of new linkage staff * Assist in identifying training needs for the Linkage program * Attend weekly Linkage meetings and provide feedback as to how to improve program and outreach efforts * Attends trainings to increase knowledge of STDs, HIV and outreach strategies * Other duties as assigned Participation in AHF Meetings/Committees Weekly Linkage staff meetingsMonthly National Linkage staff meetingsMonthly Public Health Division staff meetings Supervisory Responsibilities This position has no supervisory responsibilities.
Certificates, Licenses and Registrations - Valid Certification as an HIV testing counselor.- Valid driver?s license in state of employment with proof of liability insurance.- Phlebotomy Certified preferred
