Practice Transformation Coach Job Opening in Washington, District of columbia - Zane Networks is hiring for a Practice Transformation Coach to provide technical assistance and coaching to healthcare providers under initiatives to promote the use of Electronic Health Records, Meaningful Use and other Practice Transformation efforts. This position will serve as a trusted adviser to practices participating in quality improvement and practice transformation projects. The Practice Transformation Coach will facilitate quality improvement activities through on-site and remote technical assistance that supports the Implementation Science, Population Health, and Data driven Informed Decision Making. An ideal candidate is someone who has worked in a medical practice setting to implement or coordinate, and measure progress on quality improvement projects including clinical and administrative process improvements.
This position is a part -time or full-time position with moderate local and regional travel for practice coaching services, and program-level meetings and events related to applicable programs.
Works collaboratively with the eHealthDC and ZaneNet team to outreach and document provider participation in programs
Assists with on outreach and on-boarding of participating providers
Conducts on-site assessments of ?current state? using the designated assessment tool, baseline assessments at time of practice enrollment and follow-up assessments
Develops work plans based on outcome of initial assessment; presents findings to practice and develops a plan for improvement goals
Assists practices in selecting EHR systems and determining quality measures to target for improvement based on program-specific focus and goals
Reviews and analyzes workflows to identify current or potential barriers and implement improved process into daily practice
Identifies action plans for practices to implement to improve cost, quality and the patient experience and participate in the design, development and implementation of practice
Supports practice implementation of care coordination and care management
Coaches the practice using the elements of EHRs, population health, and data-driven decision making to achieve goals and aims
Qualifications
Minimum of 3 years? experience in the ambulatory healthcare setting
BA or BS degree in nursing, public health, health IT or a related field or relevant experience supporting improvements in ambulatory healthcare settings; clinical training and/or PCMH certification preferred but not required
Strong leadership and communication skills
Ability to train or teach clinical and non-clinical staff; must be comfortable giving oral presentations
Experience in quality improvement activities and strategies
Excellent prioritization skills
Able to work autonomously
Able to travel locally
