Our agency is seeking a full-time Hospital Nurse Liaison. The Hospital Nurse Liaison
administers and coordinates total nursing care using the nursing process for patients contemplating Hospice services while in a hospital setting; Participates in teaching patients and significant others home medical management; Works in close conjunction with patient/caregiver, physician, equipment distributors, infusion agencies, ancillary disciplines and community resources to provide coordinated quality patient care.

Current RN license from the State of California.
Two years experience in Hospice.
Ability to work in a fast paced environment with multiple organizations and disciplines.
GIP coverage.
Possession of a valid California Drivers License and the ability to maintain automobile insurance.
Ability to meet requirements of CPR and other mandated programs.
