Risk Manager/Patient Safety Overview
This position is located within the Quality Management Department of the Tuba City Regional Health Care (TCRHCC). Incumbent is responsible for the management, development, administration, education of staff, and documentation of all risk management and patient safety issues for TCRHCC to include all external extensions of the hospital. Compliance with the risk management standards is required for TCRHCC accreditation by The Joint Commission (TJC) in addition to other federal, state and tribal regulations. Data generated through this program provides a basis for establishing hospital-wide policy and for identifying trends, and issues for further study, improvements and/or corrective action by directors, department heads and/or healthcare providers. Additionally, this position is responsible for the supervision and technical direction of the Patient Advocate staff and Customer Service program.
Risk Manager/Patient Safety Responsibilities
Is responsible for the management, development, administration, evaluation, and documentation of all risk management and patient safety management program for the TCRHCC.
Assists staff with the occurrence reporting system, tracks issues, analyzes and refers them to the appropriate work center for resolution.
Reviews daily House Supervisor?s reports and intensive care unit (ICU) admissions. Attends daily Safety meetings.
Tracks and reviews incident reports relating to patient safety, and has the authority to investigate verbal or written reports containing actual, questionable or potential Risk Management issues.
Is a member of the Blood Utilization Review, Case Review/Patient Benitfit Coordinator (CM/UR/PBC), Information Technology Leadership Group (ITLG)Pain, Performance Evaluation Standardization Team (PEST), Performance Improvement Committee (PIC), Safety, and identifies facility-wide issues and recommends appropriate action to the PI, Chief Quality Officer and/or Chief Executive Officer (CEO), as appropriate.
Identifies tracks and documents all Potentially Compensable Events (PCE) in the facility. Obtains Peer Review or other reviews when appropriate, and assures corrective action is taken. Maintains documentation and assures communication to and from all appropriate staff.
Researches and assesses allegations of unacceptable patient care and provides recommended solutions, implementation monitoring and follow-up.
Identifies Sentinel Events and coordinates Root Cause Analysis (RCA), Failure Mode Effects Analysis (FMEA), as appropriate, and provides reports provided to appropriate staff.
Coordinates and tracks all tort claims, works closely with the Quality Review Board and staff, Office of General Counsel (OGC), U.S. Assistant Attorney?s Office, Indian Health Service (IHS) Regional Counsel, and TCRHCC general legal counsel. Maintains tort files in a secure manner. Notifies and works with providers named in a tort claim.
Coordinates malpractice subpoenas received for TCRHCC; works with the US Assistant Attorney, Navajo Area Office, IHS Regional Counsel, TCRHCC general legal counsel and TCRHCC staff to assure strict adherence to the correct process. Coordinates depositions and interviews, including arrangements for room reservations, scheduling, document presentation and maintenance of records.
Performs chart reviews of patient records to gather, analyze and trend data, and makes recommendations to the PIPSMC.
Prepares risk management program plan, and a yearly evaluation of program effectiveness, and trends and analyzes risk management data. Additionally, provides an annual report to Senior Leadership Committee (SLC), PIC and Board of Directors (BOD).
Consults with regional counsel in the formulation and review of TCRHCC polices and procedures that could have an impact on the facility?s liability or risk control.
Provides and coordinates education for TCRHCC employees on a wide range of risk management procedures, techniques and regulations. Provides and/or arranges workshops, in-service education opportunities, and one-on-one training, as needed to educate staff.
Reviews and monitors Medical Staff policies and activities for compliance with bylaws, credentials, and liability claims (pending and paid), as related to Risk Management.
Reviews patient complaints of a Risk Management nature, ensuring closure of issues.
Formulates strategies for improvement of quality and safety and monitors outcomes.
Provides guidance and facilitates integration and communication of proactive safety plan to reduce risk within the organization. Works closely with the hospital and medical staff and communicates to Quality and Safety Committee, Medical Executive Committee and Board of Directors.
The Risk Manager develops proactive responses to patients, their families, and employees following the investigation of potentially compensable events and asserted legal actions.
Improves quality of care by analyzing claims, reviews policies and procedures, and researches topics on Risk Management, and identifies quality-of-care issues for administration and medical staff.
Directs, trains, and supervises the work of assigned staff to achieve the defined customer service goals and to ensure smooth work flow in the department. Responsible for the orientation and education of staff to ensure compliance with new and existing goals of the organization. Monitors measurable outcomes in productivity and service quality.
Assist with the processing of payroll for the staff by maintaining employee edit requests, PTO requests, etc. Maintains accurate attendance records for staff. When appropriate, relieve staff members during employee sick/vacation time. Assume on-call responsibilities to ensure adequate staffing and problem-solving
In conjunction with the Chief Quality Officer, responsible for all hiring and related personnel decision, completing routine performance review evaluations, scheduling and staffing. Assures staff is provided a work environment conducive to productivity and good health. Determines staff qualifications and competence. Develops and maintains accurate initial and annual competency checklists, and initiates completion of initial and annual competency attestation forms.
Coordinates all activities related to the Patient Registration sector. Conducts staff meetings; evaluates and addresses issues and concerns relative to daily operations of assigned areas, also provides recommendations to Director. Work closely and professionally with nursing and ancillary departments in an effort to maintain a teamwork approach

Salary Range from $94,292 to $141,438 based on experience level Education & Experience
Education:
Must have a Bachelor?s Degree in Nursing
Certifications:
A valid, current, full and unrestricted Professional Nursing License to practice nursing in any state of the United States of America, The Commonwealth of Puerto Rico, or a territory of the United States
Experience:
Must have five (5) years? experience in a healthcare setting with experience in directing and implementing major processes relating to health care issues, such as, performance improvement and patient safety issues. Experienced ability to provide in-depth reports to a variety of audiences in a succinct and timely manner. Strong writing, interpersonal communications and organizational skills is required.
