Northern Pines Mental Health Center is a private, nonprofit mental health agency located in Central Minnesota serving the counties of Crow Wing, Cass, Aitkin, Wadena, Morrison and Todd.
Northern Pines Mental Health Center is seeking a Care Coordinator to join our Children and Families team. This Full-time position will primarily be located in Crow Wing County, MN. The primary purpose of this position is to provide assistance to consumers and providers in the coordination of care. This may include resource and referral coordinating, information gathering, and data reporting. A High School diploma or equivalent education, experience in Microsoft Programs and data entry, and a valid driver?s license is required. Current, unrestricted LPN licensure in the State of Minnesota and/or an Associate?s or Bachelor?s Degree in the Behavioral Sciences is preferred. At least one year of experience working in the Behavioral or Primary Healthcare field is also preferred. Essentials functions will include but are not limited to:
Complete preliminary screenings/ risk assessments
Assist clients by providing resource and referral
Address immediate needs of consumer as identified in the level of risk. This may require connecting consumers to Mobile Crisis Outreach services, Emergency Services and/or other supports
Process client appointments according to requirements based on level of need
Communicate with internal and external providers to ensure continuity of care
Obtaining client vitals at time of comprehensive evaluation
Provide health and wellness education to consumers
Maintain program tracking of integrated treatment planning by communicating with treatment teams
Coordinate multi-disciplinary meetings to address client needs
