About the Opportunity : The Registered Nurse (RN) works next to the Clinical Supervisor and/or Branch Manager to develop and implement care plans for their clients that coordinate the clients' home care services and that follow physicians' orders. Registered Nurse (RN) plans, organizes, and directs all client care services for clients with in their assigned caseload in accordance with current standards and regulations of the state and agency. This role manages and coordinates the clinical staff of each plan of care to implement the established plan ensuring effective management of visit utilization client care and control of expenditures.
Professional Responsibilities
Schedules Admissions visits with clients/ client family members
Documents any verbal orders for the Admission or Start of Care
Supports with agency audits as needed
Performs client intake assessments including submission of required documentation for prior authorization of client services as needed based on client's pay sources
Develops a plan of care based on nursing diagnosis, client and physician input that includes medical interventions and measurable goals or outcomes
Instructs clients, family members and primary care givers as needed to insure the maintenance of or acquisition of optimal functioning level for each client Insures effective coordination of client home care services through the timely completion of required documentation as well as the timely transfer of pertinent medical data to the client's physician, therapists or other care givers
Case management, coordination of care, and medical management consulting
Support junior staff members with continued education, ADLs and medication reviews; offers networks to resources per agency, state and federal guidelines
Support clients and families with the transition from a care facility back to their home as needed to insure the maintenance of or acquisition of optimal functioning level for each client
Routinely reviews pertinent client medical data to determine the effectiveness of services in reaching maximum rehab potential
Specific Requirements
Current Registered Nurse (RN) license in good standing
Minimum of one year (RN) Home Health Care Experience or LTC Experience a plus
Oasis a plus
IDC9/10 knowledge a plus
Travel to client home environment
Prior Admissions Experience a plus
