Registered Nurse - Refugee Care Coordinator Job Opening in Boise, Idaho - The Family Medicine Residency of Idaho, Inc. (FMRI) is currently recruiting a RN - Refugee Care Coordinator. This position is full-time schedule working Mon - Fri during normal business hours. This position is based at our Emerald St. Clinic location in Boise, ID.
Vision and Purpose
The RN: Refugee Care Coordinator provides care coordination, patient education and direct care to the participants of the Refugee patient population. In addition to providing patient-centered care to FMHC patients while adhering to the RN scope of practice, the RN: Refugee Care Coordinator navigates patients through their intake process to include immunizations and the State of Idaho Laboratory requirements. The RN: Refugee Care Coordinator will triage patients and make appropriate recommendations to the provider for care and will follow up with patients and their resettlement agency if patients miss appointments or outside specialty referrals.

Refugee Care Coordinator Duties & Responsibilities
Prepare and review patient medical information for each screening day including: IOM paperwork, the CDC?s EDN system, overseas immunization records to determine validity, screening lab results for final screening appt., titer results and adult immunization schedules for adult refugee immunization appointments.
Enter State Lab results into patient flow sheets on a weekly basis.
Identify Class B patients based on IOM paperwork and develop plan of care based on health condition.
Triage patient health concerns appropriately and help manage complex refugee referrals for patient?s that establish care with FMRI after the screening process.
Review screening protocols annually with the medical director of the screening clinic, updating necessary per newest guidelines and evidence-based practice.
Maintain competence on immunization practices by attending yearly trainings and subscribing to updates from the CDC and ACIP.
Provide support to other FMRI staff regarding refugee care including: immunizations, complex referrals, resettlement and agency logistics, and interpreter questions.
Collaborate with the refugee team at Emerald Clinic and outside resettlement agencies.
Create and manage a registry for adult refugee patients for immunizations during their first 8 months of arrival in the U.S.
Enter in-house referral orders for TB Clinic refugee patient?s with a positive TB screening test and coordinate with TB Clinic to order chest x-rays and draw labs as needed.
Maintain clinic stock of adult refugee immunizations ? including ordering immunizations as needed to keep sufficient quantity on hand
Participate in orientation and trainings regarding refugees and interpreters for FMRI providers and staff.
Attend the monthly refugee community meeting, the monthly refugee team meeting and the monthly Emerald team meeting.
Standard Registered Nurse Duties & Responsibilities:

Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
Documents patient care services by charting in the patient?s health record.
Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs.
Maintains patient confidence and protects operations by keeping information confidential.
Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques.
Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.
Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
Contributes to team effort by working toward, and accomplishing, established goals.
Provides education to team members as necessary and appropriate.
Completes immunizations orders and responsible for immunization storage and handling, including availability to help with after?hour process in the event of an outage.
Works during evening clinics or events as directed.

Qualifications
Education: ADN from an accredited College of Nursing with a minimum of 3 years nursing experience will consider Licensed Practical Nurse or Medical Assistant experience; BSN from accredited College of Nursing preferred.
Experience: Minimum of two years clinical practice in a public health or primary care setting; previous experience working with multi-cultural / refugee patients preferred.
Licensure: Valid license to practice as a Registered Nurse (RN) in the state of Idaho and current BCLS certification.
Other: Meets established Credentials Committee requirements for RN scope of practice including fully credentialed, licensed and privileged as well as the ability to pass a background check.
