Clinical Navigator Job Opening in Rockford, Illinois - Assist our clients in navigating the health care system and collaborating with the health care team. The position will work closely with our clients and members promoting wellness, problem-solving and helping individuals realize their personal goals. The role includes patient outreach, assessments, and documentation. This position is part of a team focused on achieving the goals of the Case Management program.
Provides case management services to members with chronic or complex conditions.
Contacts members who are eligible for the program for enrollment and obtaining basic health history and information regarding the member.
Identifies, assesses and manages members per established criteria.
Performs ongoing monitoring of the plan of care to evaluate effectiveness.
Develops a coaching relationship with members and assists them through the process of actively working towards better health by identifying personal goals, providing support, encouragement and education.
Documents contacts, assessments and care plan progress in information system.
Responsible for supporting operational aspects of the division to meet the organization's customer requirements and satisfaction.
Coordinate care with other health-related vendors for optimal patient care.
Maintain confidentiality related to all computer programs, medical records and other data.
Requirements
Bachelor?s degree in nursing, social work or related field.
Credentialing requirement ? at least one of the following, Registered Nurse, Clinical Social Worker (LCSW), Certified Case Manager, or Clinical Professional Counselor (LCPC). Must have current license or certificate in the State of Illinois.
Three or more years of clinical case management, including experience in motivational interviewing.
Ability to ensure completion of projects and assignments.
Must have ability to work independently and meet deadlines.
Possesses, software knowledge including word processing and spreadsheets.
Highly skilled in verbal and written communication.
Able to problem solve, exercise initiative and make decisions.
Computer skills including MS Word, Excel, Access, Outlook, etc.
