Ensure compliance with agency policy and procedures as evidenced by number of complaints and/or disciplinary actions taken over the course of the appraisal year with 100% compliance.
Conduct self in a professional manner with a positive and caring attitude as evidenced by 5 peers submitting reviews with each peer providing at least 6 positive marks of agree or strongly agree out of 11 questions with 100% compliance.
Manage medication daily (signing off on medication, stocking medication, destroying medication, checking refrigerator temperature) as evidenced by monthly medication audits completed by supervisor with 99% compliance.
Administer medication according to provider instructions with no medication incident as evidenced by self report, peer report or supervisor report with 99% compliance.
Complete required training to maintain full licensure as evidenced by CEU certificates, licensure status with 100% compliance.
Service Responsibilities:
Schedule regular appointments with consumers where you should structure each visit to monitor vital signs, mental health status, and medication needs.
Intervene as needed according to protocols and standing orders.
Provide assistive services and crisis intervention as necessary and appropriate.
Provide all services in accordance with the goals and objectives stated in the treatment plan and providing feedback to the treatment team regarding necessary modifications to the plan.
Administer injections to consumers in a competent and safe manner.
Coordinate services with other Mid-Plains Center programs to facilitate integration of medical services within the continuum of services.
Facilitate needed information from consumers regarding Patient Assistance and the LB 95 program.
Documentation Responsibilities:
Ensure that all required documentation is completed in an accurate and timely manner.
Ensure that all intake information and treatments are documented accurately and timely.
Complete regular reviews of consumer charts, treatment and medication plans, and documenting such reviews according to Mid-Plains Center policy.
Treatment and Assessment Responsibilities:
Ensure that the client has had ample opportunity to participate in the formation of the treatment plan.
Assist in the development of the treatment plan by providing feedback to the process and participating in staffing reviews.
Complete a competent triage assessment interview with the consumer and gather collateral information as available and necessary to provide appropriate, high-quality care.
Provide a clinical write-up of assessments and include observations, evaluations, preliminary diagnostic information and recommendations for treatment according to industry standards.
Continuous Quality Improvement Responsibilities:
Prepare for, attend and actively participate in all required staff meetings, program meetings and training sessions.
Meet regularly with the Supervisor for individual supervision.
Actively participate in program CQI activities, including completing required surveys and reviews, conducting satisfaction surveys with consumers, working to identify potential solutions to problems, implementing solutions developed through CQI, and assisting with the evaluation process.
Actively promote a team-oriented environment by providing, accepting and acting upon constructive feedback regarding all aspects of service delivery.
Attend to additional tasks and assignments given by the Program Supervisor as deemed necessary to the functioning and success of the Mid-Plains Center programs and services.

Qualifications:
Certificate in Nursing from an accredited LPN program.
Licensed Practical Nurse in the State of Nebraska with no sanctions.
Prefer substantial experience working in a psychiatric or mental health setting providing psychiatric nursing or other behavioral healthcare services as a Licensed Practical Nurse.
General good health. Able to sit, stand, and walk to perform job duties.
Excellent communication skills, both written and spoken. Bilingual fluency in English/Spanish is desired.
Attention to multiple details in nursing practice is required.
Able to handle many interruptions during work day.
Able to hear clients, able to sit stand and walk as necessary to perform job duties
General computer skills.
