Physical Therapist - Home Health Care Job Opening in Providence, Rhode island - Visiting Nurse of HopeHealth is looking for full time, part time or per diem Physical Therapists to join our growing team. The company's service area is Rhode Island and the bordering towns in Massachusetts. As a home care physical therapist, you will provide one-on-one therapeutic treatment and care to patients in their residence using a comprehensive and patient specific care plan, innovative solutions, and technology for point of care documentation. You will have the opportunity to spend quality time with the patient, in their own home, to promote their health and independence. You have the autonomy in addition to the support and collaboration of a full team of healthcare professionals.
Specifically, this position will:
Knowledgeably and competently delivers physical therapy to patients in their homes Evaluate patient needs through ongoing assessment and revisions to care plans based on changes in patient condition Documents and charts appropriately and within specific time frames Follows physician orders and physical therapy treatment plans for patients Identifies equipment needs of patient, requesting physician orders for equipment, working with equipment companies in ordering equipment for patient, and educating patient and caregiver on new equipment Oversees and evaluates care given by Physical Therapy Assistants Regularly re-evaluate patients to identify progress towards goal achievement; reviews OASIS answers, coding and sequencing of diagnoses to ensure accuracy; obtains orders and updates Plans of Care as needed throughout the episode of care. Participates effectively as part of an interdisciplinary team to assure that patient services are delivered in a timely manner
What we are seeking:
PT licensed to practice in the State of Rhode Island. Will we pay for licensure in Massachusetts. Minimum of 1 year experience as an physical therapist Licensed driver with an automobile that is insured in accordance with state and organization requirements
