Client Care Manager Job Opening in Los Altos, California - In this role, you will be responsible for effectively closing some client inquiries, facilitating successful handoff of qualified leads to other care managers and existing client retention and satisfaction. You will take prospective client inquiries, qualifying them and assigning them to the appropriate care manager otherwise managing yourself through assessment, closure and start of care. You will also be responsible for case management on key assigned clients as well as perform field based visits based upon direction from internal care managers. Your responsibilities will include:
Qualify, nurture, assign or otherwise and close client leads
Actively manage client leads in CRM system.
Maintain all field marketing materials
Maintain a few key referral relationships as assigned (Forum, Carlmont, APFM, , etc.)
Perform case management on key clients
Do field based client visits
Plan and execute community events
The ideal candidate will have the following qualifications:
4+ years in a care management role in the healthcare field such as a home care agency, assisted living, skilled nursing, etc.
Ability to use CRM tools, Microsoft Office applications, etc.
Ability to multitask and prioritize
Ability to be effective working independently in the field and and effective in marshalling needed resources from your co workers that support your success.
Ability to comfortably and effectively interact with all levels and ages of clients and family members.
Team player that adds to the company culture and performance of the overall team through trust, competence and enthusiasm.
