Assistant Care Manager Job Opening in los angeles, California - Assistant Care Manager Non Medical In Home Care company is looking to hire a qualified and talented Assistant Care Manager. This person helps to oversee the delivery of home care services to our clients. This position is part time and will grow to full time for the right person. Duties and responsibilities specific to Clients: Needs assessments to asses clients needs. Setting up services with new clients and follows up. Ensures everything is in place. Designs Plans of Care for clients. Ensures that care plans are renewed, maintained and followed. Communicates with Clients, Families, doctors, social workers, insurance companies and other professionals. Evaluates client progress and revises care plans as needed. Tracking outcomes in the patient condition, satisfaction of the client and the family and the financial cost savings to the individual family. On daily basis calling and visiting clients to see how clients are doing as well as check up on personal assistants to ensure that services are being delivered. Functions as a customer service liaison to hear and help resolve issues of concerns. Meets with community personnel, potential clients, and family members as needed. Assist with monitoring of the assigned team performance. Meets with staff and clients on as needed basis to ensure that the needs are met on both ends. Duties and responsibilities specific to Personal Assistants: Responsible for all employment related transactions. Recruiting, interviewing, hiring, training, supervising and termination. Ensuring that all transactions are managed according to policy and procedures Oversees the staffing and scheduling functions. Becomes familiar with scheduling in order to back up staffing. Ensures personal assistants provide and maintain the highest level of care and services in accordance with policies and Procedures. Requirements: � Experience with Care Management � Human services, CAN, Social work, Nursing Homes, LVN, and Home Health settings. � Minimum (2) years working with older adults. � Knowledge of Excel, word is required. � Must possess strong organizational and interpersonal skills. � Must be sensitive to the needs of older adults.
