POSITION SUMMARY : Incumbent is a graduate of an accredited nursing program, who holds Louisiana state licensure as a Licensed Practical Nurse. Responsible for patient check-ins and check-outs, collecting vitals and triaging urgent patients and phone calls. Collects laboratory samples in clinic as ordered, and assists with scheduling patients? specialty appointments. Provides support to practitioners and is a member of the Care Team.

This position requires cooperation with the Agency?s Quality Improvement Program. This includes actively participating in all required trainings, and assisting in creating a culture of quality improvement at the Agency. Cooperation with the quality program will be an element considered as part of all performance evaluations.
This position requires cooperation with the Agency?s Compliance Program. This includes immediately reporting any concerns, actively participating in all required trainings, following HIPAA regulations, and assisting in creating a culture of compliance at the Agency. This also includes compliance with the written standards of the Compliance Program, and all applicable federal and state laws. Cooperation with the compliance program will be an element considered as part of all performance evaluations.
This position requires cooperation with CrescentCare Compliance policies and procedures that support its status as a ?deemed? entity under the Federal Tort Claims Act (FTCA), specifically related to covered services, health center patients and FQHC-approved service delivery sites. CrescentCare provides guidance and training to assure that all practitioners are aware of services covered under its approved scope of services, the sites at which services may be provided, and the factors that define a CrescentCare patient. FTCA coverage requires that practitioners provide care within their defined scope of practice and as privileged by CrescentCare.
Failure to comply with the Agency?s Compliance Program, which may include the failure to report any conduct or event that potentially violates any laws or the Agency?s written compliance standards, will be met by the enforcement of disciplinary action, up to and including possible termination.
ESSENTIAL PERFORMANCE STANDARDS
Maintains clinical competency, as demonstrated in providing and directing patient care.
Performs functions within scope of practice
Triages patients and phone calls for urgent appointments.
Collects patients? vitals and records chief complaint at patient check-in.
Assists with patient check-outs and scheduling patient specialty appointments and diagnostic tests as ordered by provider.
Collects samples such as urine, blood, and stool specimens, in the Clinic as ordered, and prepares specimens for transport to lab.
Instructs patients in home-collection of urine, stool, and sputum specimens, when appropriate.
Administers and documents Purified Protein Derivative (PPD), as ordered.
Documents results.
Provides education to patient and patient?s personal support system in areas such as disease management, safer sex, etc.
Documents services performed for medical record keeping, compliance and reporting purposes.
Maintains order in labs and exam rooms, including stocking supplies. Notifies RN or RN Supervisor when supplies are needed.
Reviews provider orders, lab requests or follow-up needs with patient.
Documents observations in the patient?s medical record as requested by the Care Team.
Maintains standard precautions for infection control.
Maintains nursing logs as required by policies and procedures.
Promotes teamwork, good interpersonal relations and communication among staff at the healthcare setting at the local level.
Participates in Health Fairs, employee and volunteer health programs, immunization campaigns, and home visits as appropriate
Assumes additional responsibilities as directed by supervisor.
POSITION REQUIREMENTS
Education, Work Experience, Knowledge & Skills
Licensed Practical Nurse without restrictions
BLS / ACLS certifications as necessary
Experience with populations required
Valid Louisiana Driver?s License
Ability to work flexible hours
HIV/AIDS knowledge preferred
Spanish and English-speaking (bilingual) preferred
WORK CONDITIONS
Work is office-based
Working hours are usually 9am to 5pm but Licensed Practical Nurse may be expected to work earlier or stay later during busier periods.
Work with a diverse population as served by a Federally Qualified Health Center.
Standing and/or sitting for extended periods of time.
Able to use a computer to chart patient information in the Electronic Health Record.
Physical ability to participate in training sessions, presentations, and meetings.
