Case Manager/ Case Management Assistant Job Opening in Schenectady, New york - Ingersoll Place is special. But what truly makes Ingersoll Place special are the wonderful people who call it home. We are looking for a motivated individual, to fill our Case Manager position. The position would be Full-time, 40 hours a week, Monday through Friday and the occasional weekend. The individual we are seeking should have knowledge of Assisted Living Residences, various services of the elderly population, and be able to effectively communicate with families, doctors and residents.
Principal Responsibilities:
Coordinate the management of residents? health care needs with the primary physician and act as the liaison with other health care providers and with family members. This includes ensuring that all follow-up physician appointments have been scheduled and attended, all physician orders and requests have been received as required, and any required paperwork or services that need to be provided prior to an appointment are completed.
Review shift-to-shift notes, incident reports, physician orders, hospital discharge paperwork, outside provider notes, etc. to perform appropriate and proper follow-up and documentation.
Maintain accurate resident records, and maintain confidentiality of these files. This includes ensuring that all medical evaluations are current and complete, and in accordance with facility policy and DOH regulations.
Ensure continued compliance with the policies and procedures of the community as outlined in the Resident Care Policy and Procedure Manual.
Meet customer service standards in a friendly, helpful, courteous manner with resident satisfaction a priority.
Complete Individualized Service Plans (ISP) every 6 months for each resident as well as conduct Care Plan Meetings.
Coordinate with Resident Care Director that all resident care needs are met and assist with transitions from alternate levels of care (such as hospitals, rehabilitation centers, etc.), acting as a liaison for the resident in coordination with paperwork and communications.
Residents:
Assist the Resident Care Director, or perform as directed assessments as related to the physical and personal care needs of applicants for admission.
Assist in determining residents? needs at a time of change in condition.
Determine ongoing physical and personal care needs of residents. Communicate changes to Resident Care Director, and actively participate in suggesting changes to resident?s care/service plans.
Communication:
Develop and maintain effective working relationships with team members
Monitor Shift-to-Shift report; follow up as needed with the Resident Care Director.
Qualifications:
Meet qualifications required by the Department of Health to attain DOH approval.
Must have a Bachelors? Degree in Social Work with one year of Social Work experience or an Associate?s Degree in Human Service with 3 years Social Work experience. Will consider LPN with relevant knowledge and Long-Term Care or Assisted Living experience.
Experience providing care and medication assistance to the elderly population.
Case Management experience and experience working with elderly population.
