Harbor Homes is currently seeking 3 Full-time SSVF Case Managers for the following 3 locations:
One located in Northern NH (Lebanon, Littleton or Plymouth)
One located in Nashua, Manchester or Concord
One located in Dover, Portsmouth, Hampton or Exeter
This position will provide case management services to veteran households to include, assisting veterans and their families with creating an Individual Service Plan; securing stable permanent housing; and applying for financial benefits such as SSI, SSDI, VA Benefits, Food Stamps and long term supports. You will also be responsible for conducting assessments and outreach efforts, maintaining daily documentation and communication log, auditing case files, assisting clients with transportation, when necessary, maintaining positive communication with Veteran Administration and completing all necessary documentation for the VA.
Bachelor?s degree in human service field required. Master?s degree preferred. Excellent communication skills required as well as knowledge and expertise in social work, mental health and/or human services. Must possess a valid driver?s license. Background in working with Military preferred. Veterans encouraged to apply.
