Care Navigator Job Opening in Douglas, Arizona - Position Summary:
A Health Care Navigator is a specially trained behavioral health professional (BHPP) or behavioral health technician (BHT) dedicated to the delivery of 24/7 support and rehabilitation services to members in their homes and communities.
Under the agency?s Clinical supervision structure, the Health Care Navigator (HCN) will provide behavioral health paraprofessional assistance in the areas of member or family rehabilitation and support services to minimize member/family crises, help prevent member hospitalization or placement in residential settings, and promote community and school integration. The Health Care Navigator will help coordinate and lead rehabilitation and support groups, activities, and curricula designed to improve life skills, parenting skills, and teach behavior management cues in group settings or individually in homes or other appropriate settings with individual members as assigned. Duties may include coordination of community support resources beneficial to children, adults and families with behavioral health problems. The Care Navigator will work with member and family, clinical team, and others in order to identify unmet needs and necessary community, school or family support or programs and services. Works with Clinical Liaison to develop and support an individualized service plan based on member?s needs, cultural preferences, circumstances, and diagnosis. May include work to support community education, public information and social marketing campaigns and other agency activities as determined.
Minimum Qualifications To Perform Job:
High School Diploma or GED. Excellent communication skills, including ability to respond to members with patience, objectivity and a nonjudgmental attitude. Demonstrated ability to problem-solve. Knowledge of, or ability to learn, community resources. Ability to work well in a team, good interpersonal skills, and positive attitude. Ability to follow clinical and office protocols and procedures. Ability to multi-task Computer literate. Customer service, clinic or center, or social service experience preferred. Communication skills, including ability to respond to members with patience, objectivity and a nonjudgmental attitude. Demonstrated ability to problem-solve. Knowledge of, or ability to learn, community resources. Ability to work well in a team, good interpersonal skills, and positive attitude. High level of patience, empathy, and courtesy are required to deal with members and their problems. Excellent listening skills are required to effectively understand the problem that is being presented verbally. Ability to judge severity of problems and use discretion in obtaining services or support as needed. Requires only moderate supervision. Ability to use personal computers and database skills desired. Knowledge of HIPAA, and other state and federal regulations governing healthcare practices.
