Meets with patients and their families as directed to explain the services offered by Mesilla Valley Hospice and the various components of the hospice benefit.
Answers patient/family questions and explains hospice services in such a way as to ensure compliance with informed patient consent.
Facilitates early patient admission to hospice by overcoming family objections and other barriers to ensure patient?s early and barrier-free admission to Mesilla Valley Hospice.
Obtains informed written consent by patient or their designated legal medical power of attorney for hospice services as requested. Ensures that all required admission paperwork is signed and dated appropriately.
Coordinates with MVH admissions department and the referral source to ensure a prompt and smooth admission for patients.
Gathers critical information by performing the initial assessment that enables hospice to treat the patient?s immediate care needs and documents this information in Brightree by the end of the workday.
Completes the initial assessment in the location where hospice services are to be delivered.
Ensures that the patient meets Medicare and agency eligibility requirements for admission to hospice services, documents that eligibility, and ensures that the patient level of care is appropriate.
Ensures the initial assessment includes the patient?s immediate physical, emotional, and spiritual status related to the terminal illness and documents comorbidities.
Reviews current medications with patient/family upon admission and in cooperation with patient/family determine continuation or discontinuation of initial medications. Documents retained medications in Brightree.
Develops initial plan of care based on the information gathered from the initial assessment ensuring that it includes the collaboration requirement PIO and at least one other PIO as identified by the patient/family.
Provides emotional support and education to patients, their families, and their caregivers.
Manages patient?s initial medications to ensure symptom control. This includes but is not limited to titration to long acting meds as appropriate, coordinating with physicians and pharmacists and establishing current medication profile in Brightree and Vida.
Collaborates with physicians to coordinate patient admission and care throughout the admissions process until the patient?s care is assumed by the IDT and documents the collaboration.
Reviews hard chart when patients are admitted to ensure that all information is reflected in Brightree (i.e., dates of any falls, medication allergies, existing conditions, communicable diseases, etc).
Ensures that the plan of care reflects the goals of the patient and family and that patient/family understanding of the plan of care is documented.
Ensures that all plans of care are individualized, reflect the current status of the patient/family, and that the plans are a direct link to the needs of the patient/family as identified on the initial assessment.
Ensures documentation paints the picture of the patient?s current condition.
Documents all patient-related contacts, including phone calls to all members of the IDT.
Ensures that documentation on every patient visit and interaction is completed on a timely basis through bed-side charting and ensures that documentation includes data elements required for measurements of outcomes.
Ensures that appropriate interventions as identified on the plan of care are implemented in a timely manner and that the interventions are in compliance with all applicable standards of care.
Participates in a coordinated plan to visit area physicians and other area healthcare providers such as hospitals, nursing homes, assisted living facilities, personal care agencies, etc. to promote awareness of Mesilla Valley Hospice and its services, increase patient referrals, and facilitate a smooth admissions process for patients.
Assists in the smooth admission of patients by serving as a resource for admissions staff and/or medical records staff. Pick up signed doctor?s orders and/or other paperwork as needed when admissions are pending.
Troubleshoots and assists with conflict resolution with all referral sources and facilitate effective communication between referral sources and MVH.
Pronounces death of admitted patients under the authority of the Office of the Medical Investigator and prepares all required documentation.
Maintains strict confidentiality and compliance with HIPAA regulations for all information.
Follows guidelines for standard and contact precautions.
Monitors consumption and appropriate use of medications, supplies and equipment to ensure effective and efficient use of agency resources.
Ensures that Brightree forms are completed within 24 hours.
Participates in clinical on-call rotation for holidays.
Participates in staff training, mandatory meetings, and development of educational programs.
Actively participates in events that are designed to provide education to the community at large and/or events designed to raise funds for the organization as requested.
Responsible for building and maintaining a culture of personal accountability and service excellence for both internal and external customers.
Acts as improvement team member, participating in quality assessment and performance improvement activities as needed.
Identifies and reports areas for improvement and concerns for safety of patients, family and staff and suggest alternative ways of intervening that may improve outcomes based on knowledge and familiarity of situations.
Recognizes that quality improvement is part of everyone?s job responsibilities and stay abreast of performance improvement projects and their outcomes.
