The following position is open in the Case Management Department reporting to Rebecca Short: Clinical Documentation Improvement (CDI) Specialist/Case ManagerFull time ? 80 hours/pay
Major Responsibilities:
Reviews the medical record for accurate and complete documentation of all relevant diagnoses, procedures, and ancillary treatments.
Clinically evaluate how the health record translates into coded data, including review of provider and other clinician documentation, lab results, diagnostic information, and treatment plans.
Communicate with providers either through discussion or in writing regarding missing, unclear, or conflicting health record documentation, and clarify the information as warranted.
Educate providers about identification of disease processes that reflect SIO (Severity of Illness, complexity and acuity in order to facilitate accurate application of code sets.
Communicate with appropriate healthcare team members to promote accurate and complete documentation of diagnoses and/or procedures in the health record that have direct bearing on SOI.
Fills role of Utilization Review RN Case Manager 20-30 percent of the time regarding patient insurance, clinical reviews, compliance issues, etc
Other duties as assigned.
Requirements:
: Successful candidate must be a graduate of an accredited school of nursing and have a current Ohio RN licensure with five (5) years of acute care experience. Bachelor?s Degree in Nursing preferred. Previous experience in Clinical Documentation Improvement is highly desired. Must have excellent verbal and written communication skills, analytical thinking and problem solving with strong attention to detail. Must have working knowledge of Medicare, Medicaid, Social Security, and entitlement programs.
