Patient Engagement Specialist Job Opening in Lakewood, Washington - The Patient Engagement Specialist will serve as a key member of the Patient Engagement Team, addressing the needs of our most medically and socially complex patients. The Patient Engagement Team is a developing initiative to meet patients ?where they are at,? build supportive relationships which will address their social needs, and ensure better engagement in their medical and behavioral health care.
The Patient Engagement Specialist leads efforts to locate and engage new and previously hard-to-engage patients. He/she does this through research to identify accurate contact information for these individuals and through field-based activities, such as home-visits. This individual works to overcome language and cultural barriers and engages caregivers and/or household patients, as applicable.
Responsibilities:
Persistently forging trusting relationships and engages individuals not readily interested in health care services.
Utilizing communication skills such as motivational interviewing to gain agreement from Patients to participate in the onboarding or care planning process.
Using internal and external resources (such as claims, referrals, and pharmacy data) to identify accurate, successful modes of communication with unreachable or hard-to-reach patients.
Outreaching directly to patients.
Engaging patients in the most appropriate and effective setting (e.g., community, hospital, pharmacy etc.).
Providing education to new patients regarding Concerto, the onboarding process and the role of the primary care.
Actively managing an assigned set of patients that are determined to need lighter-touch, ongoing support to self-manage their own health conditions and maintain adequate engagement with their care team.
Participating in and supporting the effective planning, development, execution, and evaluation of patient engagement plans.
Assisting patients in navigating the health system and connecting with helpful community resources.
Utilizing established workflows and collaborates with internal stakeholders such as: care center care teams; field team; ancillary team patients; administration and operations staff.
Participating in collaborative team meetings and patient care planning.
Traveling to patients? homes, community locations, various agencies, and other outreach destinations.
Travel between care center locations may be required (Lynnwood, Renton, and Lakewood Care Centers)
Qualifications:
Bachelor's Degree; or AA degree or equivalent work experience in health care, or health education.
Minimum 2 years experience working in outreach or in the community with people who have medical and behavioral health conditions, social and interpersonal complexities impacting their lives.
Experience working in a community-based, multi-cultural setting.
Highly enthusiastic and confident communication style capable of motivating others.
Communication skills, such as reflecting, active listening, and paraphrasing.
Ability to investigate leads for patient outreach and engagement opportunities.
Knowledge of community resources, health and social service systems in the local community, and skill in establishing working relationships with community partners with appropriate support.
Intermediate written and verbal communication skills, including ability to communicate effectively and with patients and family members who speak English as a second language.
Basic computer literacy to comply with department needs and expectations (i.e., inputting information into electronic health records; basic use of excel spreadsheets).
Ability to work a flexible schedule, including periodic outreach duties evenings, nights, and weekends.
Ability and means to travel as needed in a timely manner within King, Pierce and Snohomish Counties, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
Must be at least 21 years of age with a valid driver's license and clean driving record.
