Reports To: Director of Primary Care Operations
FLSA Status: Hourly, Non-Exempt
Location: Administration (will travel to other clinics as necessary)
Salary : $17.30/hr + DOE
SUMMARY
This position is part of a multi-disciplinary care team to help proactively manage and stratify the healthcare needs of patient panels at Neighborhood Health Center.
Essential Job Duties & Responsibilities
Perform outreach and engagement with assigned patients with goal of increasing sustainable engagement.
Monitor engagement trends and provide recommendations to leadership on most effective engagement methods.
Aggregate and analyze care gap reports from both internal and external entities
Utilize population registries to identify gaps in care (i.e., missed immunizations, AWV or CRC screening)
Develop outreach plan to address gaps in care.
Integrate Oregon Health Plan redetermination data and identify best methods for patients to maintain coverage.
Monitor APCM engagement to maximize populations who receive care STEP or visit within 8 quarters.
Monitor leakage of APCM patients to identify abnormal trends in patients accessing care outside of NHC.
Monitor and identify population trends in meeting quality metrics and work closely with QI team to improve performance.
Use risk stratification mechanisms to monitor panel size and complexity to achieve balance of patients across teams.
Monitor risk stratification and empanelment processes and elicit provider and staff feedback.
Identify patients who could potentially be best served by other Care Team provider (i.e. behaviorist, or clinical pharmacist)
Proactively manage and monitor pertinent clinical information regarding the team?s assigned panel of patients.
Collaborate proactively with all team members and with a patient-focus to facilitate and maximize patient health outcomes.
Coordinate multi-disciplinary patient care conferences for high risk or complex patients as needed.
Communicate patient needs to other team members in a timely and effective manner.
Act as primary communication conduit between care team and other resources, both within NHC and with outside agencies.
Communicates with patient directly to discuss preventive care needs and refers patients with medical concerns to appropriate clinical staff.
Serve as a medical home quality improvement team member; assist with improvement of workflows through PDSA cycles and measurement of quality indicators.
Track progress for assigned clinic on population performance of key clinical quality metrics.
Schedule patients for preventive care needs and follow up to ensure completion of services, as appropriate.
Performs other duties as assigned.
QUALIFICATIONS
Education and/or Experience
At least two years relevant experience in a healthcare setting, outpatient primary care and care of patients with chronic disease preferred. Experience as a Certified Medical Assistant with an interest in population health management. An individual with a Bachelor?s degree with an interest in healthcare may be considered.

Ability to answer and utilize multiple line telephone and standard office equipment.
Ability to learn and adapt to changing information and/or policies and procedures.
Must be able to display sensitivity to the population and patients served.
Must be able to demonstrate a record of reliable attendance and punctuality.
Ability to proficiently use EPIC Electronic Health Record preferred.
Requires basic knowledge of medical terminology.
PHYSICAL ENVIRONMENT
While performing the duties of this job, the employee will be frequently required to sit, walk, and stand; will regularly use fingers to feel, grasp, handle or reach. Must be able to type and read for extended periods of time. Occasional bending, kneeling or stooping may be required.
While performing the duties of this job, the employee may be exposed to blood borne pathogens or potentially hazardous chemicals. The noise level in the work environment is usually moderate.
