We are currently seeking talented individuals to fill the position of Clinical Case Manager based out of our office on 7th St & Indian School but servicing the East Valley (20th St to 101)
*Bilingual (Spanish speaking) preferred but not required.
This position provides case management and coordination of services to children and their families to enhance treatment goals and effectiveness. This individual will also carry a case load size up to 190.
Provides a minimum of 25 contact hours per week of case management
Completes ISPs, Crisis Plans, Strength and Culture Discoveries
Completes accurate and timely documentation and billing for all contacts and meetings
Assists in maintaining, monitoring and modifying covered services
Completes telephone or face-to face interactions with clients, families or other involved parties for the purpose of providing services for enhancing a persons functioning and independence.
Assists in locating necessary resources, community and informal supports, other than covered services to meet basic needs.
Communicates and coordinates care with the family, behavioral and general medical providers, community resources, and other involved supports including educational, social, judicial, community and other State agencies.
Skills/ Requirements
Bachelor?s or Master?s degree in behavioral health (B.H.) related field; or Bachelor?s or Master?s degree in non-BH related field AND 1 year of BH work experience; or Associate?s degree AND 4 years of BH work experience; or High School diploma and 6 years of BH work experience.
Bilingual English / Spanish Preferred
Must be credentialed and privileged within 90 days of hire. Must meet minimum qualifications for a Behavioral health Technician
Must be able to demonstrate sufficient knowledge and understanding of human behavior and counseling theories.
Must have good skills in both verbal and written communications.
Must possess valid driver?s license and proof of auto insurance.
Must have CPR/First Aid certification.
Must have or able to obtain valid Fingerprint Clearance Card.
ADDITIONAL PREFERRED SKILLS:
Knowledge of Windows environment
Culturally sensitive
Experience working with poverty-level families and drug awareness.
Important Notes
The hours for this position are M-F, 10AM-7PM.
