Title: Registered Nurse/Care Coordinator/Case Manager
Responsibilities:
Completes initial patient assessment, including comprehensive medical,psychosocial, and functional assessment of the patient in office/telephone;review with provider and clinical team members.
Provides detailed education about patient?s specific chronic health conditions,including pathology, signs and symptoms, complications, and medications usedin treatment.
Assures that screening tests, immunizations, and urgent referrals are up to date;performs outreach when additional action or assistance is needed.
Utilizes interdisciplinary team approach to address opportunities to plan and coordinate care; acts in a supportive capacity to other team members in supporting patient/family member and treatment/care plan.
Identify internal and external community resources while planning to assist patient and family in developing short & long term care plans as appropriate.
Employs motivational interviewing and shared decision making techniques, and knowledge of cultural and spiritual values to support patient?s readiness for change in support of optimal disease management, action plans, and time frames.
Provides education and coaching to enhance patient?s knowledge and skills in support of self-care, disease acceptance, and understanding.
Promotes success with chronic care plans, transitions of care; acts as liaison to hospital, long-term-care, specialists and home care.
Reviews test results and tracks outcomes.
Reviews medications and works with provider/pharmacist as needed to assist with medication management and medication reconciliation.
Reviews patient risk issues and works with patient/family and team to reduce risks.
Leverages EHR and other support technologies to provide quality patient care
Qualifications:
RN from an accredited program, licensed with the State of Oregon.
3-5 years of experience in community health, public health, or chronic care setting.
Experience working with primary care providers to coordinate care and disease management; experience working with patients regarding their care coordination and disease management.
Ability to work independently and collaboratively, use sound judgement and critical thinking to assess and differentiate priorities.
CPR certification required.
