Case Manager Job Opening in Chicago, Illinois - The Thresholds Mothers Project provides comprehensive assistance to mothers with mental health challenges and their children. Parents in our program are working to provide safe, healthy and nurturing environments for their children, while also receiving support with mental illness, homelessness, and/or the effects of abuse, neglect, or other traumatic experiences, As most are also making the transition from adolescence to young adulthood, The Mothers Project focuses on the acquisition of independent living skills, in addition to providing education and support to develop healthy parenting skills.
The Case Manager serves as the primary transition facilitator by providing the coordination of treatment, treatment planning and the development and implementation of individualized service plans for youth and young adults with mental health needs. The case manager facilitates services ? like school, medical providers, family, etc. ? and also assists in skill building through coaching and one-on-one work with young adults.
ESSENTIAL DUTIES & RESPONSIBILITIES
Serves as the primary contact with collateral resources, funders, stakeholders, agencies, bureaucracies, and families to assist with the members? goals. Advocates and assists with problem solving/resolution that will help the client access and utilize support from the community or family.
Provides direct service to members through case management, group counseling, job support, crisis intervention, transition coordination, and psycho-social rehabilitation services in the community and at Thresholds locations.
Performs comprehensive member assessments, under direct supervision of a QMHP or LPHA, in order to identify client?s mental health service needs and to make recommendations for treatment.
Facilitates the development and implementation of a Thresholds Care Plan. This will require the assistance of the transition team, including the therapist, residential staff, supervisors, psychiatrist, guardian, member, and others.
Provides the primary linkage to community and Thresholds resources in order to assist members in achieving goals along the Transition Domains.
Ensures that the Thresholds Service Plan addresses the member?s needs and includes objectives and techniques for accomplishing each goal. Services and service plan must be focused on preparing the member for transition to adulthood.
Performs and participates in crisis intervention in accordance to TCI and Thresholds standards. Models best practices for crisis intervention as trained in TCI (Therapeutic Crisis Intervention) to other staff. Performs and supervises physical management when necessary, depending on program. Attends all TCI trainings in the required time frames.
Attends all court appearances, administrative case reviews, DCFS meetings, agency staffing, team meetings, treatment team meetings, and other meetings as assigned. Assures that all Thresholds procedures regarding the handling of member?s funds are followed and that both the members and the Thresholds administration are assured that all safeguards, reports and accounting meets the highest standards.
Completes all assigned paperwork in a timely fashion. Produces Medicaid compliant service notes. Completes reports for Thresholds, DCFS, ICG, DMH, courts, and others regularly and when required.
Facilitates monthly or quarterly staffing. Makes sure all collaterals are present, and notify parties in any staffing changes.
Takes DCFS CERAP training and passes the exam, if required by program.
Other duties as assigned.
MARGINAL DUTIES & RESPONSIBILITIES
May be required to work flexible schedules and be available to work on weekends or evenings
May be required to be on call.
EDUCATION:
Bachelor's degree in social work, counseling, psychology or closely related field
EXPERIENCE:
Experience working with persons diagnosed with mental illness preferred.
Ability to work as an effective team players.
Ability to document services provided in an Electronic Health Record in a timely manner.
Strong interpersonal skills.
Daily access to a personal vehicle is required. Must obtain and maintain $100,000/$300,000/$100,000 bodily injury and liability insurance. Subject to validation every six be required to provide transportation for members of the program using a Thresholds provided vehicle or personal vehicle. Must be 23 years of age and must meet all insurance carrier's requirements. Subject to annual Department of Motor Vehicles license verification.
