POSITION SUMMARY: The Access Specialist is the initial clinical point of contact for patients; families; physicians; discharge planners and other individuals seeking information on access to care. The Access Specialist is responsible for managing all Hospice Referrals. This position is responsible for ensuring that all State, Medicare, Medicaid, and Capital Hospice policies and procedures are met.
POSITION REQUIREMENTS:
EXPERIENCE: At least 1-2 years in clinical setting/medical office. Hospice experience desirable. Customer Service Experience highly desirable.
LICENSURE: LPN with current Commonwealth of Virginia Professional Nursing, District of Columbia and Maryland license, preferred .
EDUCATION: LPN-Graduate from NLN-accredited Nursing Program preferred and/or Associate degree or comparable experience in preferred . Social Work background desirable .
