Community Health Worker (Independent Contractor) Job Opening in Newark, Delaware - Quality Insights is searching for Community Health Workers (CHW) to assist in a short-term project. The CHW is responsible for assisting patients and members of the community in navigating and accessing health resources and programs. S/He will support our Practice Transformation Specialists, providers, and practices in health improvement initiatives through community and patient outreach strategies.
Key responsibilities include:
Engaging patients through follow-up phone calls to encourage them to enroll in prevention and management health programs and to schedule preventative screenings.
Referring patients to health prevention and screening program information phone numbers.
Providing basic information to patients regarding the prevention and screening programs.
Completion of proper documentation of outreach efforts in Excel spreadsheets.
The Community Health Worker must have:
Effective communication and listening skills ? bilingual a plus!
General understanding of HIPAA compliance requirements and ability to adhere to HIPAA guidelines to protect patient?s information and privacy
Ability to maintain positive and professional demeanor when engaging with patients, providers, and practices
Computer proficiency, which includes knowledge of Microsoft Office products, but Excel, is required.
Ability to travel locally within the state
Practices adherence to HIPAA guidelines to protect patient?s information and organizational privacy
Education & Experience Requirements:
Community Health Workers must have a high school diploma or equivalent. Prior experience working with patients, customer service, patient engagement, working or volunteering in community based programs, or experience with health initiatives is strongly preferred.
Work Environment & Requirements:
This is a short-term project (through December 31, 2017) with flexible scheduling and great pay ($20 per hour). Must be available to work at least 4 hour shifts between the hours of 9:30 a.m. ? 7:30 p.m. Monday through Friday. Reliable transportation is required as there is potential to drive from one site to another. A perfect opportunity for healthcare-related college students and healthcare retirees!
