Health Promotions Advocate Job Opening in Wilmington, Delaware - Definition:
Under the direct supervision of the Director of Fund Development the incumbent is responsible for identifying uninsured and underinsured patients as they present to health care establishment and conducting financial screening to determine program and benefit eligibility. This includes, but is not limited to, identifying potential payment sources, coordinating the application processes, eliminating potential barriers, linking the patient with health promotion and disease prevention programs and services and ensuring that all have a medical home.
Major Responsibilities:
Develop or utilize existing systems to identify uninsured and underinsured Delawareans as they present to health care establishments.
Utilize resources available within Henrietta Johnson Medical Center in order to maximize the number of uninsured and underinsured that are successfully referred and enrolled in social service and medical assistance programs.
Contact, screen and counsel patients and/or their family regarding financial eligibility for CHAP and/or other programs that provide free or reduced cost medical services.
Identify self-pay users of health care services.
Serve as an advocate for patients and providers by determining and meeting their needs as well as educating them on the importance of screening and being active participants in their own health care.
Implement activities that are educational and serve to encourage individuals to receive appropriate health screenings.
Identify patients that are at higher risk, and are least likely to be knowledgeable about health screening guidelines. Provide educational information and materials to these individuals.
Develop and implement new recruitment strategies for underserved communities as necessary.
Provide outreach services to inform local physicians of the services offered.
Navigate eligible participants through the financial screening and application process.
Contact uninsured and underinsured individuals to inform them of eligibility for CHAP, SFL, and other medical assistance programs.
Conduct initial screening to determine eligibility for CHAP, SFL, and other medical assistance programs.
Counsel patients on the application process and benefits of enrolling in CHAP and SFL.
Make appointments for individuals interested in scheduling a screening. Place both reminder and follow up calls to these individuals to ensure they receive the screening.
Act in the best interest of the Screening For Life and Community Healthcare Access Program.
Compile and report the data from activities
