POSITION SUMMARY
Provides chemical dependency counseling services to Sanctuary patients as part of a multi-dimensional treatment team.
PRINCIPLE ACCOUNTABILITIES
Conduct assessments in accordance with state laws and Hospital policy.
Ensure appropriate releases of information and all other intake forms are completed at the time of the assessment.
Complete a Continued Stay Review form on each patient upon completion of the assessment, develop a Master Problem List, and determine a course of treatment.
Develop an initial treatment plan with the patient within 24 hours of being assessed as medically able to participate in treatment.
Ensure that the patient meets criteria for placement in at least a Level III.7 modality, as defined by the ASAM PPC.
Facilitate group therapy and educational sessions in accordance with the daily activities schedule.
Ensure that each patient has an individual counseling session at least twice weekly.
Facilitate the transfer of a patient to a lower level of care once all objectives are achieved that need to be addressed in the inpatient program.
Maintain at least one current treatment plan on all patients currently receiving services.
Meet with treatment team at least once weekly to discuss patient?s progress toward treatment goals.
Make transportation arrangements a week prior to discharge. Appointments with outpatient providers, mental health counselors, physicians, other agencies should be made at time of admission.
Complete a Discharge Summary prior to discharge, and give the patient a copy to hand carry along with other discharge paperwork. Exceptions may be made if the patient leaves treatment abruptly or prematurely. In this case, a Discharge Summary must be completed within 24 hours and a copy of it forwarded to the patient and any other person or agency for which there is a valid release of information.
Provides supervision of a counselor trainee?s clinical experience and coordinates with nursing supervision regarding Addiction Technician duties.
Communicate with other staff members (supervisor, counselors, trainees, techs, nurses, etc.) regarding patient needs.
QUALIFICATIONS
Must have certification from the Department of Health as a Chemical Dependency Professional.
Must meet all qualification requirements outlined in the Sanctuary Personnel Manual.
Knowledge of all laws governing chemical dependency treatment, Deferred Prosecution, DOL requirements, and Department of Health regulations is required.
Knowledge of ASAM PPC is required.
