Due to our growth and expansion, which is attributed to our employees? knowledge, skills and commitment to providing outstanding patient care and customer service, Calvary health Care is currently seeking a Primary Home Care (PHC) Coordinator.
Job Description: The PHC Coordinator performs a variety of duties in the coordination of personalized homecare services and resources delivered by all participants to patients. Priorities include organizing patients care activities and sharing timely information across all caregivers, thereby achieving excellent customer service, adequate staffing, safer and more effective care for our patients. The PHC Coordinator will develop a professional and trusting relationship with our patients and service providers to ensure coordination and collaboration of services supporting positive outcomes.
Job :
Perform PAS/PHC admissions and program orientation.
Conduct day-to-day coordination of attendant services for all our patients, including attendant orientation, training, scheduling and ensuring back-up services for unfilled hours.
Auditing and solving time and attendance related issues.
Perform chart audits and resolve billing/claims issues.
Monitor and Analyze Quality Improvement issues and direct field staff per agency policy and procedures.
Efficient in EVV services.
Provide full range of services to patients, to include care coordination and collaboration, , referral to community resources and providers, as well as home supervisory visits as needed.
Maintain and Process all documentations with accuracy by due dates per policy, procedure and form guidelines.
Demonstrate ability to keep track of patient?s hospital , patients out of service area, patients suspensions, service breaks, and to prepare form H2076 with necessary patient information.
Evaluate care, utilization of resources, and maintenance of attendance tools, forms and procedures.
Perform other duties required by Supervisor.
Job :
Education: Minimum High School Graduate or GED. Associates Degree preferred
Experience: Must have at least six months experience in primary home care
Must have prior experience with EVV and working knowledge of VESTA software
Skills/Abilities: Highly organized, multi-tasking and well-developed oral and written communication skills. (Bilingual communication is a plus).
Ability to work in a team environment that requires quick turnaround and quality output.
Demonstrate sound judgment, decision-making and problem solving skills.
Ability to maintain confidentiality with all aspects of information in accordance with practice, State and Federal regulations.
Must be able to work with MS Office product, Adobe Acrobat.
General: MUST have a reliable transportation, proof of auto insurance, valid drivers license, must pass mandatory background clearances and must be ready to work full-time.
Job Type/Hours: Full-Time position (Mon-Fri) ? 9:00am ? 5:00pm
Salary: Dependent on Experience, Competitive pay/Negotiable.
