Hospice RN Case Manager Job Opening in Tulsa, Oklahoma - Physician's Choice Hospice is an Oklahoma-Licensed, Medicare certified healthcare organization and it is our mission to assist patients and their families with the inevitable symptoms of a terminal illness. Physician's Choice Hospice is currently seeking to hire Full Time RN Case Managers for our Tulsa Location, applicant must possess at least an Associates of Nursing from an accredited school.
Duties include: Case Managing hospice patients, attending interdisciplinary team meetings, plan of care updates, updating medications and medication adjustments, supervising any LPN or CHHA that assist the RN in patient care, back up On Call. Must have good transportation and be able to work with computers, Physician's Choice Hospice uses electronic medical record.
