Greater Flint Health Coalition is seeking a Project Director to join our growing team.
Overview :
The Project Director will lead the development, implementation, and operation of the Genesee Community Health Innovation Region?s Clinical Community Linkage Initiative, a component of Michigan?s State Innovation Model (SIM).
As a new program, the Genesee SIM Clinical Community Linkage (CCL) Initiative will serve as a ?hub? supporting a regional network of patient-centered medical homes, centralized care managers of Accountable Systems of Care (ASC), and Medicaid health plans. The CCL Hub will support Medicaid patients / beneficiaries that require community-based navigation and service linkages to address health related social needs in order to maintain coordinated care plans and improve health outcomes. The CCL initiative seeks to bridge the gap between clinical and community service providers, impacting over 41,000 lives in the Genesee region.
The Project Director will ensure the Clinical-Community Linkage Hub connects patients to a variety of community and social service resources, overseeing a team of social workers, nurses, and/or community health workers based at one of three specialty hubs. The CCL Director will be responsible for coordinating service delivery between Genesee County?s six Medicaid health plans, three SIM Accountable Systems of Care, 64 SIM-designated patient-centered medical home practices, two specialty CCL Hub partner agencies, and multiple community agencies. Daily tasks will require a skill set related to operations, project management, data analysis, communications, planning, stakeholder engagement, policy/procedure development, staff hiring & training, supervision, business agreements, and coordination. This position will work closely with the GFHC?s President & CEO and Vice President of Programs.
Primary Responsibilities :
Provide project management to the Genesee SIM Clinical-Community Linkage Hub to achieve the outcomes of the Genesee Community Health Innovation Region?s local operational plan for the Michigan SIM project.
Work closely with the region?s Accountable Systems of Care and SIM-designated patient centered medical home practice managers to receive and address referrals, complete community linkages, and follow up reporting the Genesee CHIR?s CCL Hub will provide to Medicaid patients.
Directly supervise all CCL Hub staff, including community health workers, social workers, and registered nurses. Ensure procedures are followed consistently while promoting teamwork and optimizing outcomes.
Oversee the CCL Hub intake system and the CCL Hub?s case load, delegating specialty Hub referrals, determining intervention strategies, and assigning to internal staff as appropriate.
Manage project timelines, documentation, database, and deliverables while maintaining alignment with multi-sector partner objectives and project work plan requirements.
Oversee the development and maintenance of CCL Hub policies and procedures, as well as practice training and support manuals.
Maintain ongoing communication with community stakeholders and multi-sector partners on the development and implementation of the CCL Hub with attention to continuous quality improvement.
Work with GFHC leadership, external evaluator(s), and other staff and program stakeholders to design and oversee ongoing program evaluation.
Develop and maintain good working relationships with key people at selected state, county, and community organizations and agencies.
Work with GFHC leadership in writing and managing program reports as well as additional program grants.
Work with the Vice President of Programs to convene and facilitate provider and advisory committee meetings.
Provide committee staffing leadership, ensuring responsiveness to project goals as well as partner requirements, inputs, and concerns. Collaborative process includes timely material preparation, agenda development, program/grant development, work plan execution, and the completion of required committee follow up resulting from consensus decision making.
Participate in advocacy for improved access and quality of healthcare for low-income and uninsured (or underinsured) individuals, by attending selected governmental agency meetings, legislative sessions/meetings, and community organization meetings.
Oversee the day-to-day budget of the project.
Support the mission, vision, values and key strategies of the Greater Flint Health Coalition and development of collaborative relationships with the organizations that interface with the program.

Qualifications & Experience Required :
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Minimum five years demonstrated experience, preferably in community health or clinical improvement.
Master?s degree in healthcare, public health, social work or related field strongly Bachelor?s degree with a combination of relevant experience is equivalent.
Broad knowledge of public health, healthcare delivery, health policy, and/or healthcare strategy.
Program/project management experience with the ability to develop project plans, coordinate project work, and complete projects on time and within budget.
Ability to manage an interdisciplinary project team on a daily basis, ensuring team outcomes are met and documented in alignment with project plans and procedures.
Ability to handle projects to completion, meet required deadlines, properly handle confidential materials, and work independently by finding and using information to problem solve and make decisions.
Ability to multi-task and handle various simultaneously while adhering to timelines.
Detail and problem solving oriented, and possess strong organizational skills.
Excellent interpersonal, written, and oral communication skills and an ability to present self in a courteous, tactful, and professional manner that exemplifies strong customer service.
Ability to establish positive interpersonal relationships with staff and colleagues, as well as external community for networking and partnership building.
Conflict resolution skills to respond to inquiries or complaints from stakeholders, regulatory agencies, customers, or community members. Able to listen to others, maintain confidentiality, and retain calm/professional demeanor.
High level of independent thinking and judgment, decision-making, and skill working with complex systems and executive management.
Ability to analyze complex or diverse information; use of information to identify and resolve challenges in a timely manner and develop alternative solutions.
Experience forming strategic alliances and partnerships to advance an agenda.
Training or experience with organizational development, systems change, and/or collective impact concepts.
Experience in health equity, public health, and/or social determinants of health, preferred.
Strong written and oral communication skills.
Proficient in Microsoft Office Suite ? Outlook, Word, Excel, PowerPoint, Access as well as database software.

Job Status :
Full Time, Salary; Monday through Friday, 8:30 a.m. to 5:00 p.m. on average with regular early morning meetings
