TEMPORARY SERVICE COORDINATOR Job Opening in santa barbara, California - TEMPORARY SERVICE COORDINATOR Tri-Counties Regional Center is a private non-profit organization which provides supports and services for children and adults with developmental disabilities living in San Luis Obispo, Santa Barbara and Ventura Counties so that they may live fully and safely as active and independent members of our community. We are looking for a TEMPORARY SERVICE COORDINATOR for our Community Placement Program (CPP) to serve individuals newly placed in the community from Developmental Centers. The CPP SC provides consultation, advocacy, resource information and service coordination to these individuals. Anticipated caseload for this position should be 20 to 35 cases, and there is considerable driving required. CANDIDATES MUST HAVE A Master?s or bachelor's degree in human services field or a related area, with 3 years of applicable experience. DO NOT APPLY IF YOU DO NOT MEET THESE REQUIREMENTS. Primary Job Functions: Conducts strength-based assessments which communicate an understanding of the unique issues facing family members. Involve consumer and family in all aspects of person-centered planning and support activities. Design, document, implement and monitor services and supports in a timely manner. Facilitate planning team progress, identifying outcomes, needed services and supports. Develop and maintain a network of vendors and community agencies. Provides relevant information and training about developmental disabilities, services and resources. Participates on IDT (interdisciplinary teams), including participation with IPP planning and transition planning. Advocates for individuals to reside in the least restrictive environment per regulations in the Lanterman Act, to assist with identifying needed services to continue to reside in the least restrictive environment, and advocate for provision of such services, even if this may be against conservators wishes, per the Lanterman Act. Effectively communicates, verbally or written, with individual, staff and family on a minimum quarterly basis or as requested. Documents all interactions with individuals, family, staff and those involved with the individual in T19 notes. Monitors court re-commitment dates, including 6500 and In re Hop (court commitments) and to follow up with attorney to ensure timely compliance with these and other legal matters. Updates the individual?s chart with any information or pertinent legal documents on a individual following review of information. Skill Requirements: ? Knowledge of developmental disabilities ? Awareness of entitlement and benefit programs ? Ability to access service delivery systems and operations ? Awareness of public benefit programs ? Problem-solving ability ? Verbal and written presentations to groups ? Ability to work independently ? Team collaboration skills Minimum Qualifications: ? Master?s or bachelor?s degree in human services field or a related area, with 3 years of applicable experience. ? Familiar with Community and Supported Living facilities ? Familiar with Dual-Diagnosis and/or Mental Health issues ? Valid California driver?s license and reliable, insured vehicle or equivalent means of transportation ? Able to spend 30% ? 50% of work time traveling. Overnight stays infrequent. ? Desire strong documentation skills, including knowledge of Medical Waivers santa barbara ca california
