Healthcare Engagement Manager Job Opening in Detroit, Michigan - Healthcare Engagement Manager (HEM) will utilize population health planning, account and contract management expertise to implement the Health Engagement Teams (HET) objectives and outcomes to reform primary and behavioral health care, integrating health systems and health care providers throughout Wayne County and beyond. Ensuring capacity to drive meaningful change and integration of services, employing best practices in measurement and performance improvement, and is comfortable utilizing data to drive improvement.
Works with the Manager Healthcare Integration in optimizing the operational and programmatic excellence of the services provided to the consumers being engaged, encompassing comprehensive primary and behavioral health care and treatment.
Collaborates with the Community Based Providers including the physical and behavioral health providers to identify key measures in quality and access to care, monitoring costs of care and treatment.
Ensures that HET have what they need to effectively do their jobs; training, data, and effective relationships with the various providers.
Engages in regular "rounds" at all HET meetings to observe, mentor, coach the front line staff in a manner that creates a culture of accountability and fosters continual improvement.
Assist with day-to-day operations of programing.
Assists in the gathering and coordinating of all data tracking and monitoring through various sources, including, but not limited to Smart Sheets, DASH and provider information.
Performs assignments as directed by the Manager Healthcare Integration regarding the management and operation of the Physical and Behavioral HET.
Acts as consultant to the providers (Physical and Behavioral) regarding the operations of the HET program model, benefits of consumer participation in integrated care and monitoring and management necessary to assure optimal benefit for the consumer.
Knowledge, education, training, skills, and experience in a multi-disciplinary approach to physical and behavioral health in continuous quality improvement.
Knowledge, skills and experience in working in a highly diverse work environment, coordinating care and treatment for a vulnerable population.
The ability to create effective teams through strong community relationships.
Specific training in or experience with integration of care and systems.
Ability to manage time and meet deadlines.
Excellent oral and written communication skills.
Ability to remain solution-focused and respectful in all interactions with staff, coworkers, vendors, and consumers.
Excellent problem-solving and conflict resolution skills.
Knowledge of de-escalation methods or willing to be trained in these methods
Innovative thinking.
