JOB SUMMARY
Provide overall planning, direction, coordination, management and supervision of all nursing and professional services in conjunction with the Administrator. Maintains highest level of quality care in accordance with state and federal rules and regulations. Serves as an expert resource for the agency, providing leadership and guidance to the clinical supervisors overseeing the multidisciplinary teams providing direct patient care. Provides the leadership to develop, direct and implement performance improvement initiatives consisting of organization and business process design, information systems, intake, scheduling, clinical services, marketing, public relations and finance.
ESSENTIAL FUNCTIONS
Responsible for developing, planning, organizing, and directing patient care services in accordance with federal and state regulations, accrediting bodies, and recognized standards of care.
Implements policies and procedures; goals and objectives; and standards of care relevant to patient services and staff functions.
Maintains staffing levels as needed to provide patient care to meet changing demands in patient acuity and admission volumes. Ensures schedules provide proper coverage for patient care needs.
Supports and maintains a culture of safety and quality.
Directs and supports the Quality Assurance and Quality Improvement activities, including evaluation, planning, and implementation.
Assists in preparing and meeting financial targets set by the HHA?s annual budget. Contributes to the ongoing budget management and gathering of financial data in conjunction with the Administrator.
Aids in the HHA?s goal of retaining quality staff by appropriately hiring, orienting, training, evaluating and disciplining staff within established policy. Assists in the development and implementation of orientation and ongoing training programs provided for HHA staff.
Maintains a working knowledge of current Medicare, Medicaid, and third party payer requirements for home care reimbursement. Possesses an understanding of applicable state and federal regulations as applicable to home care and communicates updates to staff.
Works closely and communicates with Administrator to ensure the highest quality of care to patients, efficient operations, business development, and financial stewardship.
Communicates and coordinates with physician?s offices, payers, patients/families, outpatient services and inpatient coordinators, community resources and agencies. Promotes home care services and coordinates care across the continuum.
Maintains up-to-date clinical knowledge of methods, products, trends, research, etc., and communicates this information to staff.
Working with Administrator, supports and facilitates implementation of existing or changes to HHA?s policies, regulations, process, or procedures.
Consults with VSO and internal/external stakeholders as needed when developing solutions or addressing issues.
Working with Administrator, sets strategic and operational goals for the branch. Assists in gathering data and developing and implementing action plans to respond to trends in metrics.
PROFESSIONAL REQUIREMENTS
Adheres to dress code; appearance is neat and clean. Wears identification while on duty.
Maintains regulatory requirements, including all federal, state, local regulations, and accrediting organization standards.
Maintains patient confidentiality at all times and is accountable for privacy and security of protected health information of the HHA.
Reports to work on time and as scheduled.
Represents the organization in a positive and professional manner to both internal and external customers.
Complies with all organizational policies regarding ethical business practices.

KNOWLEDGE, SKILLS, AND ABILITIES
Able to communicate effectively in English, both verbally and in writing.
Electronic medical record keeping experience
Proficiency in using Microsoft Office, specialized software, office equipment, and equipment used in the delivery of patient care.
Ability to plan, coordinate, direct and supervise the health services of the HHA in compliance with federal and state regulations and accrediting bodies.
Competency in nursing theory, techniques, and practice, with a particular emphasis in community health.
Possesses an understanding of the OASIS data collection and reporting process.
Develops goals, priorities, programs with contemporary knowledge of patient- centered care, value-based care and the continuum of care.
Committed to continuous improvement, clinical excellence, high degree of patient and staff satisfaction.
EDUCATION AND EXPERIENCE
Registered Nurse with a Bachelor?s Degree (preferred) in Nursing from an accredited program.
Three (3) years nursing experience. Experience in community health or home health services in the past five (5) YEARS preferred.
At least one (1) year supervisory and administrative experience.
Public Health Nurse Certificate required where applicable.
Current license from the state Board of Registered Nursing.
