Director of Nursing - Home Health Agency Job Opening in Fredericksburg, Virginia - We're opening a new branch office to serve the market!
As a full-service home health agency, we provide skilled nursing, all therapies, behavioral health and private duty personal care. We accept many payers including Medicare, Medicaid, commercial insurance, ACOs and bundles. Our unique "whole person" care model is the first of it's kind.
The Director of Nursing will be responsible for the day-to-day leadership of the branch. We are different compared to most multi-office home health agencies. We believe in autonomous branches that manage their own hiring, sales, intake, Q.A. and care delivery with local teams. The DON is in control of their office, not micro-managed. When we commit to a market, we are 100% local. The DON leverages our 12 years of experience, innovative programs, our visionary executive leadership and strong backoffice systems and operations.
Requirements:
RN or PT with significant home health management experience.
Deep clinical expertise.
Strong leader and mentor.

We provide skilled nursing, wound care and all therapy services to homebound individuals for treatment and recovery after an accident, surgery, stroke, illness, injury or other disabling event. We also provide Home Health Aides for live-in, overnight & daytime long term senior care in the home. Aide services can include personal hygiene assistance, meal preparation, medication reminders and car transportation to appointments and social activities.
Half of all individuals discharged from the hospital require additional care, treatment or rehabilitation. The problem is that care is often not coordinated and it?s divided among many different providers. The client and their family are left bewildered dealing with various people. They just want to get home and feel better. We make the process smoother and less worrisome by integrating all home healthcare services from one company. We call it whole-person care and this approach is only available from us.
We are preferred by clients, hospitals and payers with our fully integrated post-acute care solution. We are fully accountable when our client leaves the hospital by managing the care transitions and virtually eliminating unnecessary re-admissions. For most situations, we can provide the same services in the home that are performed in a Nursing Home, Skilled Nursing Facility or Assisted Living Facility. In fact, the care is better because it is 1-on-1 and the care provider is only focused on you. Home is where you want to be and we make that a reality.
To achieve our industry leading results, we believe in 24/7 local management and local intake in the markets we serve. We hire clinicians who live in the same areas as our clients. We do not use centralized call centers. This approach has earned us an A+ rating with the Better Business Bureau. All of our services and locations are accredited by JCAHO - the hospital standard. Only 15% of home health agencies receive this high standard.
We advocate for our clients in dealing with the payers: Medicare, Medicaid, commercial insurance companies, payment bundles and Accountable Care Organizations. Our clients are at the center of everything we do. We are their voice and we take that responsibility seriously.
We have developed innovative and comprehensive programs for advanced wound care featuring MIST Therapy and telewound imaging, Parkinson?s care featuring LSVT Big & Loud, Alzheimer?s care featuring BCAT Memory Tools, orthopedic pre-hab and rehab, behavioral health programs and many senior care services. Coming soon, we will be performing physician house calls to provide interim primary care in the home directly after hospital discharge and continuing until our client is able to resume visits to their regular physician.
