R/N / LMSW (HHA or Hospice Experience Required) Job Opening in Double Springs, Alabama - NurseTrek is now hiring an experienced Registered Nurse, or Licensed Master Social Worker.
There are immediate openings to do on-going case management services for healthcare members in the Double Springs, AL area and surrounding regions:
Qualifications & Requirements:
* Must have an active Alabama license with a minimum 1 year in-home case management
experience
* Must be able to work independently
* Must be willing to travel to clients homes within a 40-45 mile radius of your location
* Must have experience traveling to a client's residence
* Home Computer with Internet and Cell Phone Required (Computer and Internet
Proficiency)
* Required Company Specific Training Provided
* Experience With Computerized Charting preferred
* Must pass drug screening and background check
* Must be able and willing to complete required training and begin working within 5 days of job
offer.
This job will have the following responsibilities:
Coordination and scheduling of client visits
Completion of paperwork in a timely manner
Conducting in-home visits
Member Assessments
Identifying needs Education on Disease-Process Safety
Communication with Primary Care Physician
Assessment of needs for Community Resources
Caregiver Education
Monitoring of member and referrals to appropriate resources
Additional duties as needed
