POSITION OVERVIEW:
The System AVP ? Care Management will serve with high visibility, directing as well as administering operational, fiscal and strategic initiatives for the system-wide Case Management, Care Transition and Utilization Review programs
Oversight of 4 managers and 60+ total FTEs ? need an inspiring mentor/leader for a bright/enthusiastic team of managers and staff
Leads and provides technical resources for the design, development, acceptance, and implementation of case management standardized practices and performance standards across acute care hospitals, behavioral facility and clinics
Identifies improvement opportunities and facilitates process standardization across the system to promote improved outcomes, maximize resource utilization and improve organizational efficiences
Ensures processes and procedures adhere to standards of accrediting and regulatory agencies
Forms positive, collaborative relationships with members of the executive team, physicians, hospital staff, providers, and payers
Ensures patients receive quality, cost effective care, consistent with clinical appropriateness, regulatory guidelines established standards of care
Remains abreast of advances in the areas of healthcare technology and information management systems (HIM), while evaluating their effect on case management practice and patient care services.

POSITION REQUIREMENTS:
If RN (Registered Nurse), Bachelor?s Degree in Nursing required with Master?s degree preferred or LCSW with a MSW (Master of Social Work)
Demonstrated leadership experience that has included accountability for the design and effectiveness of care improvement, care processes, and/or transitions in care
Five (5) years of case management experience of which a minimum of three (3) years have been in a system level case / care management leadership capacity
Demonstrated ability to establish goals, vision, and overall departmental direction to include; creating and implementing departmental standards, systems, staffing plan and standardized processes; analyzing issues and creating an effective plan to address them; critically and comprehensively evaluating the department?s outcomes; identifying, collecting and analyzing data; developing educating and mentoring staff
Knowledge of external requirements such as CMS, The Joint Commission and other accreditation or regulatory agencies
Certification in Case Management (CCM or ACM) or willingness to pursue certification desired
This position requires previous multi-site facility experience
