We are in search of a Certified Medical Assistant who would serve as a vital team member and work collaboratively to provide patient-centered quality care. Role Summary: This individual is considered one of the most important members of the Care Coordination team. They will work on the phone to ensure that patients are following their physician?s care plan, as well as collaborate with our Care Coordinators to arrange for the highest level of quality patient care. In addition, they will work supporting team members of our patient-centered teams by performing administrative duties, including verifying insurance coverage, coordinating referrals, and accurately documenting in the electronic health record.
Key Responsibilities:
Regular phone contact with patients in the Care Coordination program
Verify insurance coverage for care coordination services
Liaison with Care Coordination nurses regarding patients care.
Assist in identifying patients needing additional services and help to coordinate services
Abstracting of clinical information from CSNAP and the electronic health record to reflect preventative services received
Facilitate referrals to outside providers
Schedule patients with Think providers
Send appropriate medical records to outside providers
Follow up on outcome of referrals
Accurate documentation in the electronic health record
Attendance and punctuality are requirements of the role

Other duties as assigned
Skill, Knowledge, and Experience:
2+ years of experience in a clinical setting, Family Practice or Internal Medicine ideal
Certified Medical Assistant
Certified Medicine Aide
CPR certified
2+ years of experience with electronic health record, Allscripts ideal
Proficient computer skills
