The Registered Nurse (RN) Case Manager develops, implements, and delivers the entire plan of care and services for our clients. RN Case managers coordinate clients' home care services in addition to working with physician referrals and guidelines. RN Case Managers set up their schedule with the clients and Branch Managers to ensure the best possible care for our clients.
Responsibilities
1:1 nursing with clients in their homes ,skilled nursing and non-skilled (ADLs supervision) and provide cares based on physician orders and following care plans
Case management, coordination of care, and medical management consulting
Support junior staff members with continued education, ADLs and medication reviews; offers networks to resources per agency, state and federal guidelines
Support clients and families with the transition from a care facility back to their home as needed to insure the maintenance of or acquisition of optimal functioning level for each client
Routinely reviews pertinent client medical data to determine the effectiveness of services in reaching maximum rehab potential
Performs client intake assessments including submission of required documentation for prior authorization of client services as needed based on client's pay sources
Assures agency intake processes meet applicable local, state or federal requirements in addition to agency policies and procedures
Requirements
Current Registered Nurse (RN) license in good standing in the state of application
(RN) Home Health Care, LTC or Acute experience a plus
Oasis or ICD-10 a plus but not required
