Options Case Coordinator Job Opening in Boston, Massachusetts - United Cerebral Palsy of MetroBoston (UCP) is a progressive, non-profit agency committed to advancing the independence, productivity and full citizenship of people with disability. UCP is looking for a Case Coordinator for its Options for Community Living Program. This position is 40 hours per week.
Qualifications: Bachelor?s Degree required in field related to human services. A minimum of two years in a human service related position, preferably working with individuals with developmental or physical disabilities. UCP reserves the right to accept equivalent experience and demonstrated ability in lieu of a degree.
Responsibilities: Enhance the independence of the individuals served to lead a safe and self-determining life by coordinating support services which may include: skills training, Personal Care Attendant/Homemaker services, therapy services, healthcare, and transportation. Participate in the development of support plans, emphasizing the empowerment of the persons served. Record data, write monthly, quarterly, and annual progress notes, and maintain case files in accordance with internal and external guidelines and regulations. Assume responsibilities of a surrogate to aid persons served in management of the Personal Care Attendant Program. Participate in UCP?s emergency beeper rotation and promptly respond to emergencies as needed. Candidate must have a valid driver?s license and access to a vehicle.
