Currently seeking a Social Services Director to join our dynamic management team for our 99-bed, CMS 5-Star rated long-term care facility located in Jefferson, Ohio. Our facility has an exceptional history of survey performance and quality clinical outcomes.
The ideal candidate will have previous experience in a long-term care environment and exhibit a compassionate and empathetic demeanor in caring for our residents.
Requirements and responsibilities of the Director of Social Services include but are not limited to: Knowledge of the Waiver program, PASSAR requirements, Level of Care (LOC) and Medicaid eligibility requirements, discharge coordination, care conferences, MDS assessments and quality assurance.
