Behavioral Health Case Manager/Pop. Health Advocate Job Opening in Klamath Falls, Oregon - Mountain Valleys Health Centers is looking for Behavioral Health Case Manager/Population Health Advocate for Butte Valley/Tulelake Health Centers in Dorris and Tulelake, CA. This position will perform the duties of a case manager for the behavioral health department as well as duties related to tracking population health.
The Case Manager Component will consist of working independently and together with MVHC counselors and other community agencies to provide outreach and direct case management services to help clients achieve wellness and recovery goals. They will provide short term help with problems such as transportation to appointments, insurance applications, housing, etc. for MVHC patients. They will engage in prescribed phone or personal contact with patients referred by Behavioral Health providers.
The Population Health Advocate Component will consist of creating and maintaining registries for MVHC?s patients. Population Health is defined as those activities related to tracking health outcomes and prevention activities for those identified patients. The Population Health Advocate is responsible for documentation, tracking, auditing and reporting of these measures. The Population Health Advocate is also a patient advocate to programs available to our clients within the Health Center, such as FPACT, CDP, etc.
This position will be located at Butte Valley Health Center in Dorris, CA with minimal travel to other MVHC sites. Benefits include Insurance coverage, PTO, Retirement/AFLAC plans, etc. Pay DOE, training available.
