MEDICAL RECORDS CLERK Job Opening in West Palm Beach, Florida - MEDICAL RECORDS CLERK/ INTAKE COORDINATOR needed for a busy Home Care Referral Agency located in West Palm Beach.
Medical Records / Intake Coordinator is the first contact with families, doctors and healthcare professional inquiring about the agency. They are responsible for obtaining and securing an appointment for any possible inquiries relating to patient referrals and coordinating those functions necessary to bring the referral to conclusion.
Responsibilities:
Performs and can demonstrate all essential Customer Service job functions
Performing Intakes for all new referrals
Receives and responds to incoming calls from referral sources and/or potential patients, exchanges information to identify the organization?s ability to meet the needs of the patient, on a case-by-case basis
Advises appropriate staff of patient acceptance and provides all relevant information to ensure patients? smooth transition to home care and contacts referral sources to advise of referral status, and provides to the clinical team who will deliver the services requested
Create paper file and digital file
Documents and routes all calls in a manner consistent with the organization?s policies and procedures
Conducts courtesy follow-up calls, as required by company policy and procedure
Identifies potential payer sources, verifies benefits with payer sources (as required by department organizational structure), negotiates rates and obtains initial authorizations from case managers.
Monitors and tracks referral sources? satisfaction levels
Communicates customer service issues to management, as appropriate
Maintains communication with referral sources and other interested parties, such as facility staff, physicians, and family members, as appropriate, to keep all concerned up-to-date in regard to the progression of the referral, per current agency procedures and guidance from management
Tracks pending patient referrals, ensuring all requirements for admission are accomplished
Any other duties related to position
