JOB DESCRIPTION SUMMARY
PRN Field Position in our East Valley area. Determine your own caseload! Visit times and days are scheduled directly with the patient around your existing schedule. All notes are entered electronically from your home computer (you may use one in our office if you do not have one) following established policies and procedures regarding timelines and form completion.
COMPENSATION
OT Evaluations: $95/visit
OT Re-Evaluations and OASIS Discharges: $85/visit
OT Visits and Discharges: $80/visit
Mileage Reimbursement: $0.40/mile
JOB DUTIES
Conducts appropriate evaluations.
Assesses home environment (as appropriate).
Identifies equipment needs relative to occupational function.
Identifies functional occupational defects and establishes a plan of care to improve patient?s function.
Administers therapy program utilizing specialized therapeutic technique and/or equipment.
Conference with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care for all of our home health patients
POSITION QUALIFICATIONS
Must hold a current Occupational Therapy license in Arizona and be in good standing with all regulatory boards
Current CPR Certification
Fingerprint Clearance Card
Valid Driver License
Current car insurance
Home Health experience is preferred, but not required
Must possess strong organizational and communication skills
